The bone-marrow niche — the address a blood stem cell lives at, which 26 entries in this graph invoke and none had defined

also called haematopoietic stem cell niche · HSC niche · bone marrow microenvironment · BMM

The bone marrow is not a bag of loose cells: it is an organ with addresses, and a blood stem cell only behaves like a stem cell when it sits at the right one, held there by signals from the cells around it. That address is called the niche, and leukaemia cells move into it, shelter there from chemotherapy, and remodel it so it suits them better than it suits normal blood.

The same thing, for a scientist

The haematopoietic stem cell (HSC) niche is the anatomically and molecularly defined microenvironment inside bone marrow that maintains HSC self-renewal, quiescence and retention. Four decades of work converged on a small set of cellular constituents: **CXCL12-abundant reticular (CAR)/LEPR-positive perivascular stromal cells**, the dominant source of CXCL12 and SCF, whose loss depletes HSCs [E1][E2]; **NG2-positive periarteriolar pericytes**, which specifically maintain quiescence, with cycling HSCs redistributing to LEPR-positive perisinusoidal sites [E3]; **Nestin-positive mesenchymal stem cells**, spatially paired with HSCs and with adrenergic nerve fibres [E4]; **osteoblastic cells at the endosteum**, the first proposed niche [E5]; **sinusoidal endothelium**; **megakaryocytes**, covered in `cell-type-megakaryocyte`; **adipocytes**, predominantly negative regulators; and **sympathetic nerve fibres**, which impose a circadian rhythm on CXCL12 and thus on HSC egress [E6]. Direct two-photon phosphorescence measurement in live mice showed marrow pO2 is low everywhere (<32 mmHg) and **lowest in deep peri-sinusoidal regions (~9.9 mmHg, 1.3 percent), with the endosteum *less* hypoxic** because it is perfused by small arteries [E7] — which inverts the textbook hypoxic-endosteal-niche picture. Every one of these components has a documented interaction with KMT2A-rearranged leukaemia, and each is written as a separate entry in this beat.

Layman first: what the niche is, where it is, and what it does for a healthy blood cell

Every drop of blood in a child's body is replaced constantly — red cells last about four months, some white cells only hours. All of it is made in the bone marrow, the soft tissue inside the hollow centre of bones, by a very small number of blood stem cells. A single one of them can make every kind of blood cell, and can make copies of itself.

The trouble with a stem cell is that it is useless if it does either job too enthusiastically. Divide constantly and it exhausts itself and the supply fails; never divide and nothing gets made. Something has to tell it which to do and when. That something is not inside the cell. It is the place the cell is sitting in.

A niche, in ecology, is the slot an organism occupies in its environment. A stem-cell niche is the same idea shrunk to a few cell-widths: a specific spot in the marrow, surrounded by specific neighbouring cells, releasing specific molecules the stem cell can feel. Sit in the niche and you are held still, fed and protected. Leave it and you start dividing, or you wash out into the bloodstream and leave the marrow altogether.

Where is it? Bone marrow is threaded with blood vessels. Small arteries (arterioles) bring blood in under pressure; the marrow drains through wide, leaky, slow-flowing vessels called sinusoids. Wrapped around both are stromal cells — the supporting, non-blood cells of a tissue. At the edge of the marrow cavity, lining the inner surface of the bone itself, is the endosteum, a layer of bone-building cells (osteoblasts). Most blood stem cells sit against a vessel wall, touching a stromal cell; some sit near the bone surface. Running alongside the arterioles are nerve fibres of the sympathetic nervous system — the fight-or-flight system — which turn the niche's signals up and down on a daily clock.

What does it do for the cell? Four things, each with a named molecule:

  1. Holds it there. Stromal cells pour out a chemical called CXCL12 (also called SDF-1). Stem cells carry the matching receiver, CXCR4. CXCL12 is a homing beacon; block CXCR4 and stem cells wash out of the marrow into the blood within hours. This is not a laboratory curiosity — it is how the drug plerixafor is used in clinics to collect stem cells from donors.
  2. Keeps it alive. Stromal and endothelial cells make stem cell factor (SCF/KITL), without which stem cells die 2.
  3. Keeps it still. Signals from periarteriolar cells 3, from megakaryocytes and from TGF-beta hold the cell in a resting state (see cell-state-quiescence). A resting cell is a protected cell: nearly every leukaemia drug works best on cells that are dividing.
  4. Sets a rhythm. Sympathetic nerves lower CXCL12 at a fixed time each day, releasing a pulse of stem cells into the circulation 6.

The reason a cancer graph cares is point 3. The same address that protects a healthy stem cell protects a leukaemia cell — and the leukaemia does not merely squat there, it rebuilds the place. That story is in mechanism-leukaemia-remodels-its-own-niche.

The granularity rule applies: each of these is one thing and gets one entry. This entry is the map, not a substitute for them.

What this entry deliberately does not claim

Almost all of the cellular anatomy above is mouse work, built on conditional knockouts and reporter alleles with no human equivalent. The human marrow niche has been described mainly by imaging and single-cell transcriptomics, not by functional deletion, and the correspondence between mouse Lepr+/Nestin+/NG2+ populations and human stromal subsets is an active argument rather than a settled mapping.

Second, the field has repeatedly overturned itself. The endosteal/osteoblastic niche was the consensus from 2003 5; by 2012 conditional deletion showed that SCF from osteoblasts is dispensable while SCF from endothelium and LEPR+ perivascular cells is essential 2. Treat 'the niche' as a set of experimentally separable activities, not as a place with a fixed address.

Third, this entry claims nothing about infants. Every functional study cited is in adult mice. Since the disease of interest here presents at a median age of a few months, and since the developmental stage of the niche demonstrably changes the lineage of MLL-rearranged leukaemia (mechanism-niche-developmental-stage-sets-leukaemia-lineage), assuming the adult niche describes the infant marrow would be an error this graph should not make.

How this could bite us back
  • The niche is not a leukaemia-specific structure. Every pharmacological attempt to disrupt it also disrupts the compartment the patient depends on to recover their own counts, which is precisely why plerixafor is used clinically to mobilise normal stem cells rather than to kill leukaemic ones.
  • 'Niche protection' is easy to invoke and hard to falsify, and this graph has already rejected it once as a menin-inhibitor resistance mechanism because no primary study supports it in that setting. An entry treating the niche as a general explanation for treatment failure would repeat that error.
  • Expanding the osteoblastic niche with PTH reduces CML stem cells but INCREASES MLL-AF9 AML, so niche-directed interventions can be actively harmful in a genotype-specific way.

  1. What does the infant bone-marrow niche look like? Every functional study cited here is in adult mice, and the disease in this graph presents in the first year of life.
  2. Are CAR cells, LEPR+ perivascular cells and Nestin+ MSCs three populations, two, or one seen through three reporters? The conditional-deletion results disagree in a way usually blamed on cre drivers rather than biology, which is not a resolution.
  3. Where is the hypoxic compartment? Direct pO2 measurement says peri-sinusoidal and explicitly not endosteal; the MLL-AF9 metabolic-imaging literature says leukaemia-initiating cells home to a hypoxic endosteal niche. These cannot both be right.
  4. Does human paediatric marrow contain periarteriolar NG2+ pericytes at all, and can they be identified in a diagnostic trephine biopsy, which is the only human marrow tissue routinely available?

Induced deletion of CXCR4 in adult mice severely reduces HSC numbers and increases sensitivity to myelotoxic injury without impairing expansion of more mature progenitors; most HSCs contact CXCL12-abundant reticular (CAR) cells, which surround sinusoidal endothelial cells or lie near the endosteum, making CXCL12-CXCR4 signalling essential for maintaining the quiescent HSC pool.

    What this evidence cannot show

Mouse, induced conditional deletion. CAR cells were defined by CXCL12 reporter intensity, a definition later work largely subsumed into the LEPR-positive perivascular population; whether CAR and LEPR+ cells are one population under two names is still argued. Read: abstract only.

Evidence that argues against this (1)
    • CXCR4's requirement in leukaemia is not the same requirement as in normal HSCs, and the difference undermines a simple 'CXCL12 holds cells in the niche' reading. A CRISPR screen in syngeneic MLL-AF9 AML found Cxcr4 deletion eradicated leukaemia in vivo WITHOUT impairing homing to the bone marrow, and CXCL12 itself was dispensable for leukaemia development in recipients; loss of CXCR4 signalling caused oxidative stress and differentiation. In this disease CXCR4 can be essential for a reason unrelated to the chemokine gradient that defines the niche. Read: abstract only. Ramakrishnan R et al. 2020

Stem cell factor (SCF/KITL) is expressed primarily by perivascular cells throughout the bone marrow; HSC frequency and function are unaffected when Scf is deleted from haematopoietic cells, osteoblasts or Nestin-cre-expressing cells, but HSCs are depleted when Scf is deleted from endothelial cells or from leptin-receptor-expressing perivascular stromal cells, and most HSCs are lost when it is deleted from both.

Mouse, Scf-GFP knock-in plus conditional deletion. The negative result for Nestin-cre is contested by the group that defined Nestin+ MSCs, partly on the grounds that Nestin-cre and Nestin-GFP label different populations; this is a live methodological dispute about cre-driver specificity, not a settled refutation. • Nestin-positive MSCs were reported in the same period to be an essential HSC niche component, with in vivo depletion rapidly reducing marrow HSC content and progenitor homing, and with Nestin+ MSCs containing all bone-marrow CFU-fibroblast activity. The SCF-deletion result places the maintenance factor elsewhere; both cannot be the whole story. Read: abstract only. Mendez-Ferrer S et al. 2010

Whole-mount confocal imaging with computational three-dimensional association analysis shows quiescent HSCs associate specifically with small arterioles preferentially found in endosteal marrow; these arterioles are ensheathed exclusively by rare NG2/CSPG4-positive pericytes distinct from sinusoid-associated LEPR-positive cells; activating the HSC cell cycle redistributes HSCs from NG2+ periarteriolar to LEPR+ perisinusoidal niches, and conditional depletion of NG2+ cells induces HSC cycling and reduces long-term repopulating HSCs.

    What this evidence cannot show

Mouse. The core claim is a spatial-statistical association; the causal arm is NG2+ cell depletion, which removes a pericyte population with functions beyond HSC support. 'Preferentially found in endosteal bone marrow' means the arteriolar and endosteal descriptions overlap anatomically, a frequent source of confusion in this literature. Read: abstract only.

Evidence that argues against this (1)
    • The assignment of quiescence to the arteriolar niche competes with the finding that megakaryocytes, a haematopoietic rather than stromal cell, sit non-randomly next to HSCs and that their depletion specifically abolishes HSC quiescence. The niche activity 'maintain quiescence' has at least two independent claimants with independent depletion evidence. Read: abstract only. Bruns I et al. 2014

Mesenchymal stem cells identified by Nestin expression constitute an essential HSC niche component: they contain all bone-marrow CFU-fibroblastic activity, are spatially associated with HSCs and with adrenergic nerve fibres, highly express HSC-maintenance genes that are selectively downregulated on enforced mobilization or beta-3 adrenoreceptor activation, and their in vivo depletion rapidly reduces marrow HSC content and progenitor homing.

Osteoblastic cells are a regulatory component of the HSC niche in vivo: mice engineered to express an activated PTH/PTHrP receptor in osteoblasts have increased osteoblast numbers, high jagged-1, increased HSC numbers with in vivo Notch1 activation, and PTH injection into wild-type mice increases stem-cell number and markedly improves survival after bone marrow transplantation.

Osteoblastic cells regulate the haematopoietic stem cell niche · Calvi LM, Adams GB, Weibrecht KW, Weber JM, Olson DP, Knight MC, et al. · Nature 2003
    What this evidence cannot show

Mouse, gain-of-function genetics plus pharmacology. Later conditional-deletion work showed osteoblast-derived SCF is dispensable for HSC maintenance, so this paper establishes that osteoblasts CAN regulate the niche, not that they are required for it. Read: abstract only.

Evidence that argues against this (1)
    • The same osteoblastic manipulation has opposite effects in different leukaemias. Osteoblast-specific activation of the PTH receptor ATTENUATED BCR-ABL1-driven CML-like disease (a 15-fold decrease in LSCs) but ENHANCED MLL-AF9-driven AML in mouse transplantation models, attributed to opposing effects of increased niche TGF-beta1. Expanding the osteoblastic niche is therefore not generically protective, and in the genotype this graph is about it would be expected to make things worse. Read: abstract only. Krause DS et al. 2013

Circulating HSCs and progenitors oscillate on a circadian rhythm in antiphase with bone-marrow Cxcl12 expression; the cyclical release and the Cxcl12 oscillation are driven by core clock genes acting through circadian noradrenaline secretion from sympathetic nerves innervating the marrow, transmitted to stromal cells by the beta-3 adrenergic receptor, reducing nuclear Sp1 and rapidly downregulating Cxcl12.

Haematopoietic stem cell release is regulated by circadian oscillations · Mendez-Ferrer S, Lucas D, Battista M, Frenette PS · Nature 2008
    What this evidence cannot show

Mouse; mice are nocturnal, so the phase does not transfer to human clock time without reversal. The receptor here is beta-3; in leukaemia the pro-leukaemic adrenergic signal was mapped to beta-2 and explicitly not beta-3, so the normal and malignant adrenergic circuits are not the same circuit. Read: abstract only.

Evidence that argues against this (1)
    • In an MLL-AF9 AML model the adrenergic signalling that promotes leukaemogenesis was transduced by the beta-2, not the beta-3, adrenergic receptor on stromal cells, and the direction of the biology is inverted: leukaemia destroys sympathetic fibres rather than exploiting their normal rhythm. A therapeutic inference drawn from circadian beta-3 physiology would be aimed at the wrong receptor. Read: abstract only. Hanoun M et al. 2014

Direct in vivo two-photon phosphorescence lifetime measurement of local oxygen tension in live mouse bone marrow gives absolute pO2 below 32 mmHg throughout despite very high vascular density, with the lowest values (about 9.9 mmHg, 1.3 percent) in deep peri-sinusoidal regions and the endosteal region LESS hypoxic because it is perfused by small, often Nestin-positive arteries; these values change markedly after radiation and chemotherapy.
