CAR cell (CXCL12-abundant reticular) / LEPR+ perivascular stromal cell — the cell that makes the signal holding blood stem cells in the marrow

also called CXCL12-abundant reticular cell · CAR cell · LEPR+ perivascular stromal cell · Prx1+ mesenchymal cell (overlapping population) · adipo-osteogenic progenitor

These are branching support cells wrapped around the blood vessels of the bone marrow, and they produce most of the chemical beacon (CXCL12) that keeps blood stem cells from drifting away. In leukaemia they are the cells that feed and shelter the cancer's own stem cells.

The same thing, for a scientist

CXCL12-abundant reticular (CAR) cells were defined by CXCL12 reporter intensity as the stromal cells most HSCs are in contact with, surrounding sinusoidal endothelium or lying near the endosteum; induced CXCR4 deletion in adults collapses the HSC pool, establishing the axis they serve [E1]. Conditional-deletion work subsequently localised the essential maintenance factors to **leptin-receptor (LEPR)-expressing perivascular stromal cells and endothelium**, and the field now treats CAR and LEPR+ cells as largely the same population identified by two markers [E2]. The KMT2A-relevant result is specific and recent: in an MLL::AF9 AML model, **CXCL12 produced by Prx1-positive mesenchymal cells, but not by mature osteolineage cells, maintains leukaemia stem cells**, and it does so by controlling energy metabolism and redox balance — Cxcl12 deletion caused ROS accumulation and DNA damage in LSCs and impaired their ability to perpetuate leukaemia on transplantation, a defect **attenuated by antioxidant therapy**, with evidence the phenomenon is conserved in human patients [E3]. So the cell that shelters the leukaemic stem cell is a mesenchymal perivascular cell, and the shelter it provides is metabolic.

What the cell is

A reticular cell is a stellate, branching stromal cell that forms a mesh through a tissue. CAR cells are the marrow's reticular cells that express CXCL12 at very high level. Sugiyama and colleagues found that most HSCs are in physical contact with one, and that these cells sit both around sinusoids and near the endosteum — meaning that a single stromal population spans the two anatomical compartments the field had been treating as rival niches 1.

They are not a terminal cell type. CAR/LEPR+ cells are adipo-osteogenic progenitors: they are the source of marrow adipocytes and of osteoblasts, which links this entry to cell-type-bone-marrow-adipocyte and cell-type-osteoblast as differentiated descendants rather than as unrelated neighbours.

Naming caution. The literature calls overlapping versions of this population CAR cells, LEPR+ cells, Prx1+ (Prrx1) mesenchymal cells, and in some papers simply MSC. These labels are reporter alleles, not proofs of identity, and this entry does not assert that they mark exactly the same cells. Where a specific result is cited, the marker actually used is stated.

The leukaemia result, and why it is not the protection story you would expect

The CXCL12/CXCR4 axis is routinely invoked as the pathway by which stroma shelters leukaemic cells from chemotherapy. Viñado and colleagues asked where the CXCL12 comes from and what it actually does, in an MLL::AF9 model. Their answer: Prx1+ mesenchymal cells, not mature osteolineage cells, and the protection is metabolic rather than simply anti-apoptotic — deleting Cxcl12 caused reactive oxygen species to accumulate and DNA damage to appear in leukaemia stem cells, impairing serial transplantation, and antioxidant therapy attenuated the defect 3.

That last clause is the interesting one, and it cuts against the simple reading. If the niche's contribution can be replaced by an antioxidant, then what the niche supplies is a metabolic buffer, not an inviolable sanctuary. It also means that a CXCR4 antagonist given with an antioxidant would be expected to work less well, which is a concrete, testable and clinically relevant prediction that nobody appears to have tested.

Separately, a CRISPR screen in the same MLL-AF9 background found CXCR4 essential to the leukaemia independently of CXCL12: Cxcr4 deletion eradicated leukaemia without impairing marrow homing, and CXCL12 was dispensable for leukaemia development in recipients 4. Read together, 3 and 4 say that the ligand matters (from Prx1+ cells) and that the receptor's essentiality does not require the ligand. The field has not reconciled these, and an entry that reported only one of them would misrepresent the state of knowledge.

How this could bite us back
  • CXCL12 from this cell maintains normal HSCs as well as leukaemic ones, so blocking the axis mobilises the patient's remaining healthy stem cells at the same time — which is exactly how plerixafor is used deliberately in donors.
  • If antioxidants substitute for the niche's metabolic support, then supportive care that raises antioxidant capacity could unintentionally rescue leukaemic stem cells from a niche-disrupting drug.
  • The receptor may be essential for a reason the ligand is not involved in, so a ligand-blocking strategy and a receptor-blocking strategy are not interchangeable even though the field treats them as such.

  1. Does a CXCR4 antagonist lose efficacy when given with an antioxidant? [E3] implies it should, and that would be a decisive test of whether the niche's contribution to KMT2A-rearranged LSC survival is metabolic.
  2. Are the CAR, LEPR+, Prx1+ and Nestin+ labels marking one population or several? Every functional claim in this entry depends on an answer nobody has supplied.
  3. Has anyone identified the human paediatric equivalent of a CAR cell in a diagnostic marrow trephine, and does its abundance at diagnosis predict measurable residual disease?
  4. Does KMT2A::AFF1 infant ALL depend on Prx1+ mesenchymal CXCL12 as MLL::AF9 AML does? The stromal-protection data in infant ALL are real but the cellular source of the protective signal was never identified.

Most HSCs are in contact with CXCL12-abundant reticular (CAR) cells, which surround sinusoidal endothelial cells or lie near the endosteum; induced deletion of CXCR4 in adult mice severely reduces HSC numbers and increases sensitivity to myelotoxic injury without impairing expansion of more mature progenitors, establishing CXCL12-CXCR4 signalling as essential for maintaining the quiescent HSC pool.

    • Conditional deletion of the other key maintenance factor, SCF, showed the requirement lies with LEPR-expressing perivascular cells and endothelium, and that deletion from osteoblasts or Nestin-cre cells was dispensable. This does not refute the CAR result but it relocates the functional definition of the niche cell from a CXCL12 reporter to a LEPR-driven cre, and the two populations are assumed rather than shown to be identical. SCF is expressed primarily by perivascular cells throughout bone marrow; HSCs are depleted when Scf is conditionally deleted from endothelial cells or from leptin-receptor-expressing perivascular stromal cells, and most HSCs are lost when deleted from both, whereas deletion from haematopoietic cells, osteoblasts or Nestin-cre-expressing cells has no effect.

    • Nestin-GFP-positive MSCs were independently shown to contain all marrow CFU-F activity, to express HSC-maintenance genes highly, and to be required for marrow HSC content and progenitor homing when depleted in vivo. In an MLL::AF9-induced AML model, CXCL12 produced by Prx1-positive mesenchymal cells but not by mature osteolineage cells maintains leukaemia stem cells in the marrow by modulating energy metabolism and redox balance; Cxcl12 deletion causes reactive oxygen species accumulation and DNA damage in LSCs and impairs their ability to perpetuate leukaemia in transplantation, a defect attenuated by antioxidant therapy, with data suggesting the phenomenon is conserved in human patients.

    What this evidence cannot show

Mouse syngeneic MLL-AF9; 'CXCL12 dispensable' rests on the recipient genotype, which does not exclude leukaemia-cell-autonomous or alternative-ligand stimulation. Evidence that argues against this (1)
    • The KMT2A-rearranged human data point the other way on the ligand's relevance: primary infant MLL-rearranged ALL is differentially protected by stroma, stroma-conditioned medium alone is sufficient, and CXCR4 antagonism with AMD3100 markedly enhanced drug efficacy in xenografts. A CXCL12-independent mechanism in murine myeloid disease does not transfer to that setting. Read: abstract only. Sison EA et al. 2013
