MLL-AF9 leukaemia destroys the nerves that run its own niche — a mechanism class this graph had nothing on
also called leukaemic niche remodelling · AML-induced sympathetic neuropathy · niche co-option
Leukaemia does not simply move into the bone marrow and take up space: it damages the nerves that control the marrow's support cells, which converts the neighbourhood from one that suits healthy blood stem cells into one that suits the leukaemia. Because the damage is to nerves rather than to the cancer cells themselves, it is in principle repairable with drugs that act on nerve signalling.
The same thing, for a scientist
In an MLL-AF9 mouse AML model, disease development **disrupts sympathetic nervous system nerve fibres in the bone marrow and abolishes the quiescence of Nestin-positive niche cells**, driving expansion of phenotypic mesenchymal stem and progenitor cells primed for osteoblastic differentiation **at the expense of the HSC-maintaining NG2-positive periarteriolar niche cells**. The leukaemogenic adrenergic signal is transduced by the **beta-2, and explicitly not the beta-3, adrenergic receptor** on stromal cells of leukaemic marrow [E1]. The authors' reading is that sympathetic neuropathy is a mechanism by which the malignancy **co-opts** the microenvironment, and that malignant and healthy stem cells have separable mesenchymal niche requirements. A 2025 follow-up letter shows marrow neuropathy is not specific to MLL-AF9: it is present in JAK2V617F MPN, NRAS-G12D pre-leukaemic myelopoiesis and p53-deletion lymphoid disease, is present in human MPN and AML marrow, involves severe ultrastructural fibre damage on electron microscopy, and — the clinically loaded point — **is itself induced by chemotherapy** [E2]. Independently, MLL-AF9 AML remodels the mesenchymal compartment transcriptionally, expanding Ebf2-positive MSPCs with **reduced Cxcl12**, and deleting Ebf2+ cells **accelerates** leukaemia, so the native mesenchymal niche is partly suppressive before it is remodelled [E3].
What the founding experiment actually shows
The brief that dispatched this beat named PMID 25017722 as a lead to verify rather than repeat. It was fetched and read, and it says what the brief said it says, with two details worth keeping exact.
First, the direction: AML causes the neuropathy. The title's word is 'leukemia-induced'. This is not a pre-existing host susceptibility that permits leukaemia; the disease develops, and as it does, sympathetic fibres are disrupted and Nestin+ niche cells lose quiescence 1.
Second, the consequence is a swap, not a loss. The MSPC compartment expands, but the expansion is of cells primed for osteoblastic differentiation, and it happens at the expense of NG2+ periarteriolar cells — the population independently shown to maintain HSC quiescence (cell-type-ng2-periarteriolar-cell). So the remodelled niche is not merely a damaged normal niche; it is a niche whose composition has been shifted away from the one that holds normal HSCs still 1.
Third, the receptor is beta-2. The paper tests both and reports that the leukaemogenesis-promoting adrenergic signal goes through beta-2 and not beta-3 on stromal cells 1. That matters because normal circadian niche physiology runs through beta-3 (cell-type-bone-marrow-sympathetic-nerve). The normal and the malignant adrenergic circuits use different receptors, and an intervention designed from the physiology would target the wrong one.
It generalises, and chemotherapy does it too
A 2025 letter from a different group extends marrow neuropathy beyond MLL-AF9 to NRAS-G12D-driven pre-leukaemic myelopoiesis and to p53-deletion lymphoma/lymphoblastic leukaemia, documents severe ultrastructural damage to sympathetic fibres by electron microscopy in NRAS-G12D mice and in AML patient marrow, and shows that chemically reinforcing the neuropathy promotes myeloid bias, reduces HSPC numbers, reduces mesenchymal stromal cell numbers and produces morphologically abnormal marrow vessels 2.
The sentence in that letter with clinical consequences is its last: chemotherapy induces neuropathy of the HSC niche, and chemotherapy is first-line for exactly the diseases in which the neuropathy is pathogenic. If the niche damage caused by the leukaemia is pro-leukaemic, and the treatment causes the same damage, then induction chemotherapy is plausibly deepening the very lesion that favours the residual disease. This is the same shape as the megakaryocyte finding already in the graph, where chemotherapy inverts a quiescence-enforcing niche signal into an expansion signal (cell-type-megakaryocyte). Two independent niche compartments, same structural claim: treatment remodels the niche that decides what residual disease does next.
Note what is and is not established. That chemotherapy induces niche neuropathy is asserted in 2 with a citation to prior work; this entry records it as reported in a letter rather than as a result of 2's own experiments, and it has not been demonstrated in KMT2A-rearranged infant leukaemia in any species.
The mesenchymal compartment is remodelled transcriptionally, and was suppressive to begin with
A separate MLL-AF9 study mapped the stromal side directly. AML infiltration expands Ebf2-positive MSPCs with reduced Cxcl12 and enhanced production of more differentiated mesenchymal progenitors; in vivo fate-mapping shows Ebf2+ cells participate in building the leukaemic niche; and Ebf2+ cell deletion accelerates AML, implying the native mesenchymal niche suppresses the disease before it is remodelled. AML dysregulated Angptl1, Cxcl12, Kitl, Il6, Nov and Spp1 in marrow MSPCs, partly before the massive expansion of AML cells. Primary patient AML cells induced similar alterations in xenografted mice 3.
Two things follow. The remodelling is early, so it is a candidate for a pre-relapse readout rather than only a late consequence of tumour burden. And 'niche' is not a synonym for 'protection': deleting a niche population here made the leukaemia worse, which is the direct opposite of the protective-niche framing.
Where this stands relative to the rest of the graph
This is a mechanism class, not a single finding: the leukaemia actively alters the tissue it lives in, and the alteration is functional rather than incidental. The graph previously held nothing on it. mechanism-kmt2a-r-tissue-localization records where KMT2A-rearranged cells sit and that stroma protects them; it does not record that the cells rebuild the stroma.
The evidence base is entirely MLL-AF9 murine AML, which is a myeloid model of a disease that in infants is overwhelmingly B-lymphoid. Nothing here has been shown for KMT2A::AFF1 or in infant ALL, and the rejection ledger (ana-marrow-niche-rejected-candidates) records that the sympathetic/adrenergic intersection with the whole KMT2A literature is two records, both cited here.

- Beta-blockade to reverse niche remodelling would act on a receptor expressed throughout the cardiovascular and respiratory systems of an infant, and beta-2 blockade specifically risks bronchospasm in a population with frequent respiratory infection during neutropenia.
- If the native mesenchymal niche partly suppresses leukaemia (Ebf2+ deletion accelerated AML), then a drug that disrupts the stroma to 'release' leukaemic cells could remove a restraint instead of a shelter.
- Neuropathy is produced by chemotherapy as well as by leukaemia, so any niche-repair agent would be fighting the effects of the treatment it is given alongside.
- Nerve fibres regenerate slowly if at all; a niche lesion that is structural rather than signalling may not be reversible on any timescale useful to a patient in induction.
- Does KMT2A::AFF1 infant B-ALL cause marrow sympathetic neuropathy? Every result here is myeloid and murine, and the infant lymphoid case is the one this graph exists for.
- If chemotherapy induces the same niche neuropathy the leukaemia induces, does induction therapy leave behind a niche that is more permissive to residual disease than the one it started with? This is testable in a xenograft with serial nerve-fibre imaging around a treatment cycle.
- Is beta-2 blockade (propranolol is cheap, ancient and paediatric-licensed for haemangioma) active against KMT2A-rearranged leukaemia in vivo, and would it be antagonistic or synergistic with a menin inhibitor?
- Ebf2+ MSPC deletion accelerated AML while the niche is usually framed as protective. Which mesenchymal subsets restrain KMT2A-rearranged disease and which support it, and does the balance shift during treatment?

In an MLL-AF9 mouse AML model, neuropathy of the sympathetic nervous system promotes leukaemic bone-marrow infiltration: AML development disrupts SNS nerves and the quiescence of Nestin-positive niche cells, expanding phenotypic mesenchymal stem and progenitor cells primed for osteoblastic differentiation at the expense of HSC-maintaining NG2-positive periarteriolar niche cells, with the leukaemogenesis-promoting adrenergic signal transduced by the beta-2 but not the beta-3 adrenergic receptor expressed on stromal cells of leukaemic bone marrow.

Mouse, retroviral MLL-AF9 myeloid model; no lymphoid or KMT2A::AFF1 arm, and infant disease is not modelled. 'Phenotypic MSPCs' is a flow-cytometric definition, and the osteoblastic priming is inferred from marker and differentiation-gene expression rather than from fate. The adrenergic-receptor assignment rests on pharmacology plus genetics in a system where beta-2 and beta-3 agonists have overlapping off-target profiles. Read: abstract only.
Evidence that argues against this (1)
- Marrow sympathetic neuropathy is not specific to MLL-AF9 or even to leukaemia: the same lesion is present in JAK2V617F MPN, in NRAS-G12D pre-leukaemic myelopoiesis and in p53-deletion lymphoid disease, and the authors state explicitly that chemotherapy itself induces neuropathy of the HSC niche. A finding that generalises to every driver tested and is also produced by the treatment is weaker evidence for a KMT2A-specific mechanism than the original single-model report suggests, and raises the possibility that neuropathy is a shared consequence of marrow infiltration rather than a mechanism the fusion drives. Neuropathy of the haematopoietic stem and progenitor cell niche is present in multiple experimental models of haematological disease including MLL-AF9 AML, JAK2V617F MPN, NRAS-G12D pre-leukaemic myelopoiesis and p53-deletion lymphoma/lymphoblastic leukaemia, and in human MPN and AML marrow; it involves severe ultrastructural damage to sympathetic fibres on electron microscopy; chemically reinforced neuropathy promotes myeloid bias, reduces HSPC and mesenchymal stromal cell numbers and produces morphologically abnormal marrow vessels; and the authors note that chemotherapy induces niche neuropathy.

PubMed classifies this record as a Letter, so it is short-format and its methods are not fully reported in the abstract. The statement that chemotherapy induces niche neuropathy is a citation to earlier work, not a result of this letter, and is recorded here as such. The new models are NRAS-G12D and p53-deletion, neither of which is KMT2A-rearranged. Read: abstract only.

- Because the same neuropathy appears across drivers and is also produced by chemotherapy, the observation cannot by itself distinguish a mechanism the leukaemia uses from a non-specific consequence of marrow disease. The original MLL-AF9 report nonetheless supplies the causal arm the generalisation lacks, with a specific receptor and a specific niche-cell swap, so the two records pull in opposite directions on specificity while agreeing on the phenomenon. - The niche is not uniformly protective here and the direction is inconvenient for a niche-protection story: deleting the Ebf2+ mesenchymal population ACCELERATED leukaemia, which means the native mesenchymal niche was restraining the disease. The same inversion appears in the osteoblastic compartment, where PTH-driven osteoblastic expansion reduced CML stem cells but enhanced MLL-AF9 AML. Any intervention framed as 'disrupt the protective niche' must first establish which direction that particular compartment pushes in this genotype. Read: abstract only. Krause DS et al. 2013
