ITGA4 (VLA-4) and NG2 — the niche-adhesion axis of KMT2A-rearranged B-ALL, and the one approved drug that hits it

also called NG2-ITGA4 axis · VLA-4 / alpha-4 integrin adhesion · natalizumab repurposing in KMT2A-r B-ALL · CSPG4-ITGA4

Leukaemia cells stick to the bone marrow using a surface hook called integrin alpha-4, and in this genetic type of leukaemia that hook works together with a second molecule, NG2, that the fusion protein itself switches on. An antibody already licensed for multiple sclerosis blocks the hook, and in mice carrying children's leukaemia it slowed the disease and made standard chemotherapy work better.

The same thing, for a scientist

NG2 (CSPG4) is a direct transcriptional target of KMT2A fusions and a long-standing immunophenotypic hallmark of KMT2A-rearranged B-ALL, associated with poor prognosis, early relapse and glucocorticoid resistance [E1]. Its mechanism at the niche interface is now defined: **NG2 and the alpha-4 integrin subunit ITGA4 form a biological axis** in which NG2 expression promotes proliferation and migration of KMT2A-r B-ALL cells and is associated with **Rho GTPase activity in an ITGA4-dependent manner**; combined genetic ablation of both genes significantly delayed disease onset and prolonged survival in immunodeficient mice; and **natalizumab, an FDA/EMA-approved monoclonal antibody against ITGA4, delayed leukaemia progression and potentiated standard-of-care chemotherapy in KMT2A-rearranged B-ALL patient-derived xenografts** [E2]. ITGA4 is the alpha chain of VLA-4, the integrin that binds VCAM-1 on marrow stroma and fibronectin in marrow matrix, and VLA-4/VCAM-1 is the canonical adhesion arm of the niche alongside the CXCL12-CXCR4 chemokine arm. This is the strongest actionable niche-directed result in this beat: it is in **KMT2A-rearranged B-ALL** rather than a murine myeloid model, and the drug is already approved with paediatric exposure.

Why this is the most useful entry in the beat

Almost every other niche mechanism written here has one or more of three defects: it is murine, it is myeloid (MLL-AF9), or its pharmacology does not exist. 2 has none of them.

  • Right disease. KMT2A-rearranged B-ALL, in patient-derived xenografts.
  • Right molecule. ITGA4 is the alpha chain of VLA-4, the integrin that binds VCAM-1 on marrow stromal cells and fibronectin in matrix. Adhesion-mediated drug resistance through VLA-4 is a decades-old idea in myeloma and AML; this is its specific instantiation in this genotype.
  • Right drug. Natalizumab is approved by FDA and EMA for multiple sclerosis and Crohn's disease, with an established dosing and safety record including in adolescents. Repurposing is a regulatory and toxicology problem, not a discovery problem.
  • Right readout. It delayed progression and potentiated standard-of-care chemotherapy — the combination effect that a niche-disruption strategy is supposed to produce and that plerixafor did not deliver in POE 10-03 (trial-poe-10-03-plerixafor-paediatric).

The partner molecule closes a loop this graph already had open. NG2 positivity is one of the diagnostic hallmarks of KMT2A-rearranged B-ALL (concept-flow-cytometry-immunophenotype-kmt2a-r), it drives CNS infiltration (concept-cns-involvement-at-diagnosis), and it causes glucocorticoid resistance by downregulating NR3C1 through FLT3/AP-1 1. Now the same molecule turns out to work through an integrin — so the fusion's own target gene is a niche-adhesion effector. Niche dependency in this disease is not only something the environment imposes on the cell; part of it is transcribed by the fusion.

The NG2 naming hazard, stated so nobody trips on it

NG2/CSPG4 appears in this beat in two completely different roles and they must not be confused.

  1. Niche cell marker. NG2 marks the rare periarteriolar pericytes that maintain HSC quiescence in mouse marrow (cell-type-ng2-periarteriolar-cell).
  2. Leukaemic blast antigen. In the KMT2A literature NG2 is overwhelmingly a blast marker: of the 19 PubMed records intersecting NG2/CSPG4/pericyte terms with the KMT2A disease half, the great majority concern NG2 on leukaemic cells as a diagnostic marker, a prognostic factor, a CNS-infiltration effector, a glucocorticoid-resistance mechanism or a CAR-T target, and only one is about NG2+ niche pericytes.

The hazard is practical, not semantic. An NG2-directed agent — and CSPG4-specific CAR T cells for high-risk childhood BCP-ALL have been described — would engage both the blast and the periarteriolar niche cell. Whether that is a liability (destroying the HSC-quiescence niche the patient needs) or an unexpected advantage (removing a niche compartment along with the blast) is unknown and has not been asked. Targeting ITGA4 with natalizumab does not have this problem, because ITGA4 is not the pericyte marker.

ng2 dual role warningNG2/CSPG4 marks BOTH the periarteriolar niche pericyte that maintains HSC quiescence and the KMT2A-rearranged leukaemic blast; of 19 PubMed records intersecting NG2/CSPG4/pericyte terms with the KMT2A disease half, nearly all concern the blast and one concerns the niche pericyte. An NG2-directed agent would engage both; an ITGA4-directed agent would not.

How this could bite us back
  • Natalizumab's known association with progressive multifocal leukoencephalopathy arises in chronically immunosuppressed patients; a child in leukaemia induction is profoundly immunosuppressed, which is the worst possible background for that specific risk.
  • VLA-4 blockade mobilises normal haematopoietic progenitors as well as blasts, so like CXCR4 antagonism it strips the patient's own marrow of its anchor during the period it most needs to regenerate.
  • Blocking adhesion releases blasts into the circulation in a disease that presents with hyperleukocytosis; the mobilisation hazard that applies to plerixafor applies here.
  • E1 and E2 come from the same group, so the axis has no independent replication; a therapy argument built on one laboratory's pair of papers is exactly the profile this project has been burned by.
  • NG2 also drives CNS infiltration. If ITGA4 blockade impedes marrow adhesion without impeding CNS tropism, it could shift disease towards the sanctuary site that already carries the worst prognosis in this population.

  1. Does natalizumab work in INFANT KMT2A::AFF1 B-ALL specifically, and at what exposure? The PDX result does not report an infant-specific arm and natalizumab has no infant pharmacokinetic data.
  2. Natalizumab carries a class risk of progressive multifocal leukoencephalopathy on prolonged exposure. Is a short, induction-length course in a leukaemia protocol below the exposure threshold that risk depends on?
  3. PDX models use murine stroma. Does the natalizumab effect depend on human VCAM-1, and would it be larger in a humanised-niche model?
  4. Would blocking ITGA4 and CXCR4 together be additive? They are the two arms of the same niche interaction and no study has combined them in this genotype.
  5. NG2 causes glucocorticoid resistance and NG2 signals through ITGA4. Does natalizumab restore glucocorticoid sensitivity, which would be the cheapest possible combination in infant ALL because steroids are already in the backbone?

NG2 is an epigenetically regulated direct target gene of the MLL-AF4 fusion protein and its expression is a hallmark of MLL-rearranged B-ALL while being minimally expressed in healthy haematopoietic cells; NG2 negatively regulates expression of the glucocorticoid receptor NR3C1 and confers glucocorticoid resistance to MLLr B-ALL cells, mechanistically by interacting with FLT3 to produce ligand-independent activation of FLT3 signalling and downregulation of NR3C1 via AP-1-mediated transrepression.

    What this evidence cannot show

Single-group finding from the Barcelona group (`collab-menendez-bueno-barcelona`), not independently replicated as far as this beat found. NG2 positivity as an MLLr hallmark is long established; the causal chain NG2 to FLT3 to AP-1 to NR3C1 is the new and less-tested part. Cell lines and primary cells; the abstract does not report a clinical correlation of NG2 with prednisone response in a trial cohort. Read: abstract only.

    • NG2's role in this disease is not confined to, and may not be primarily, glucocorticoid resistance: the same molecule was earlier shown to drive leukaemia invasiveness and central nervous system infiltration in MLL-rearranged infant B-ALL, with NG2 markedly upregulated on CNS-infiltrating blasts and NG2 blockade almost abolishing CNS engraftment. A single molecule carrying prognosis, CNS tropism, integrin signalling and steroid resistance is doing a great deal of explanatory work, and the relative contribution of each is not established. In KMT2A-rearranged B-ALL, the alpha-4 integrin subunit ITGA4 and NG2 form a key biological axis contributing to leukaemic aggressiveness: NG2 expression promotes proliferation and migration of KMT2A-r B-ALL cells and is associated with Rho GTPase activity in an ITGA4-dependent manner; combined genetic ablation of ITGA4 and NG2 in immunodeficient mice significantly delayed disease onset and prolonged survival; and natalizumab, an FDA- and EMA-approved monoclonal antibody targeting ITGA4, delayed leukaemia progression and potentiated the efficacy of standard-of-care chemotherapy in KMT2A-rearranged B-ALL patient-derived xenograft models.

    What this evidence cannot show

2026 publication from the same group as E1, so E1 and E2 are not independent. Patient-derived xenografts in immunodeficient mice, which lack the immune compartment and carry a murine rather than human stroma - a material limitation for an adhesion mechanism that depends on stromal VCAM-1. 'Delayed progression' and 'potentiated chemotherapy' are survival-curve statements whose effect sizes are not given in the abstract. No infant-specific arm is described, and the number of PDX models is not stated. Natalizumab's mechanism here is assumed to be adhesion blockade; the abstract does not report a VCAM-1-binding readout. Read: abstract only.

    • The clinical precedent for niche-adhesion disruption in paediatric leukaemia is discouraging in a way this result has to answer. The CXCR4 antagonist plerixafor, targeting the chemokine arm of the same niche interaction, engaged its target in children with relapsed leukaemia - mobilizing blasts a median 3.4-fold, correlating with surface CXCR4 - and produced only three responses in eighteen evaluable patients, none in ALL. Target engagement in this compartment has already been shown not to guarantee benefit, and the integrin arm has not yet been tested clinically in leukaemia at all. Read: abstract only. • The same study's central finding cuts against a simple environment-determines-behaviour reading: fetal MLL::ENL-expressing lymphomyeloid multipotent progenitors are intrinsically programmed towards a lymphoid fate yet give rise to MYELOID leukaemia in vivo, which is the opposite direction from the heterochronic transplantation result in which a neonatal environment produced B-lymphoid character. Extracellular regulation is demonstrated; its direction and dominance are not. Read: abstract only. Rowe RG et al. 2019
