ENL stays after the fusion leaves — an epigenetic lesion that outlives the protein that made it
also called ENL-marked epigenetic lesion · cofactor persistence after complete menin eviction · MLLT1 retention at oncoprotein target loci
In one patient on a menin inhibitor the drug worked perfectly: both the fusion protein and menin became undetectable. The leukaemia carried on anyway, because a helper protein called ENL was still parked on the same stretch of DNA, keeping the genes running without the protein that originally put it there.
The same thing, for a scientist
In a KMT2A-rearranged leukaemia sample that lineage switched during revumenib treatment, the oncoprotein and menin were reduced to undetectable levels while **ENL (MLLT1), a transcriptional cofactor of the oncoprotein, persisted on numerous oncoprotein-target loci**, including genes of the GMP-like lineage-switching program. The authors' proposal is explicit in the preprint title and closing sentence: KMT2A oncoproteins can induce epigenetic lesions, marked by ENL, that support resistance to targeted therapies [E1][E2]. If this generalises, it is the most uncomfortable resistance mechanism in this beat, because it survives *complete* success of the drug. It is also a direct empirical test of a hypothesis already in this graph: `hyp-apparent-authority-residual-fusion-occupancy` predicts that a subset of fusion binding sites retains **fusion** occupancy at saturating drug. The only human observation available says the opposite — the fusion goes, and a cofactor stays. The site-level consequence is the same (a residual occupied set that maintains output) but the identity of what is left, and therefore what a second agent must displace, is different.
What was seen, and in how many samples
One. This entry rests on one primary patient sample, profiled as part of a 36-sample oncoprotein-occupancy survey, and it says so throughout. The observation is that in the sample that lineage switched during revumenib treatment, the oncoprotein and menin were reduced to undetectable levels but ENL persisted on numerous oncoprotein-target loci, including genes in the GMP-like lineage-switching program 1.
The reason a single observation earns an entry is the logical structure of what it shows. Every other resistance mechanism in this beat is compatible with the idea that the drug did not fully work: the drug could not bind (MEN1 mutation), a parallel input substituted (RAS), the output was re-created from other loci (PRC1.1), or a downstream step failed (TP53). Here the drug worked completely by its own definition — menin gone, fusion gone — and the target loci were still occupied and still active. No amount of better pharmacology addresses that.
Why ENL specifically, and why it is not a surprise once stated
ENL is a YEATS-domain reader of acylated histones and a component of the super elongation complex; it is a physiological partner of the KMT2A complex and also one of the recurrent KMT2A fusion partners in its own right (KMT2A::MLLT1). The graph already carries gene-mllt1 as both a canonical fusion partner and an interactome member.
That dual role is the point. A cofactor that is recruited by the oncoprotein but which then reads a histone mark rather than binding the oncoprotein has no reason to leave when the oncoprotein does. The oncoprotein writes the state; the reader maintains it. This is the definition of an epigenetic lesion in the strict sense — a heritable chromatin configuration that no longer requires the initiating signal — and it is exactly the configuration for which a protein-protein interface inhibitor is the wrong tool.
The disagreement with this graph's own hypothesis, stated so it is not lost
hyp-apparent-authority-residual-fusion-occupancy (in analogy/) predicts: at saturating menin-inhibitor concentration a reproducible subset of KMT2A::AFF1 chromatin binding sites retains fusion occupancy through menin-independent tethering (CXXC-DNA, LEDGF/PSIP1 PWWP, AFF1-SEC contacts). Its first listed falsifier is: CUT&RUN under saturating drug shows essentially complete, uniform loss of fusion occupancy, with no reproducible residual peak set.
The one human sample profiled under revumenib shows the oncoprotein reduced to undetectable levels 1. That is the falsifier condition for the fusion-occupancy half of the hypothesis, in the only relevant material anyone has produced. What it does not falsify is the broader idea the hypothesis is reaching for, that a residual occupied set maintains output at saturating drug — because a residual occupied set is exactly what was found, marked by ENL instead of by the fusion.
Two caveats before anyone treats this as settled. First, this sample is a lineage-switched sample, and lineage switching is itself associated with reduced oncoprotein levels independent of drug 1, so the undetectable oncoprotein may be a property of the switched state rather than of drug saturation. Second, 'undetectable' in a primary-sample chromatin profile is a sensitivity statement. The hypothesis should be revised rather than discarded, and this beat does not own that node — the finding is reported for the owning Queen rather than edited in.
What would make this actionable
The countermeasure class is already in the graph. Agents that evict the driver by a route independent of the menin pocket — catalytic KAT6/KAT7 inhibition, which rapidly evicts the MLL fusion from chromatin and overcomes both genetic and non-genetic menin-inhibitor resistance 3 — attack the occupancy directly rather than the interface. A YEATS-domain inhibitor or ENL degrader would be the mechanistically exact answer and is a real chemical class in development, though no study tests one against menin-inhibitor resistance. Bounded as of 2026-09-19 (raw eutils esearch): (YEATS OR ENL OR MLLT1)[Title/Abstract] AND (revumenib OR ziftomenib OR bleximenib OR "menin inhibitor" OR "menin inhibitors" OR "menin inhibition")[Title/Abstract] returns 3 records, and (ENL OR YEATS)[Title/Abstract] AND menin[Title/Abstract] returns 7. All were read. Two of the three are this entry's own source in its journal and preprint versions (PMIDs 39472576, 38234854) and the third is a review of menin-KMT2A targeting that mentions MLLT1 only as a fusion partner (PMID 39887730). None administers a YEATS inhibitor or ENL degrader to a menin-inhibitor-resistant or menin-inhibitor-treated model. The gap is real and is now a counted, dated negative rather than an unbounded one.
The cheap first step is not a drug at all. It is to run ENL CUT&RUN alongside the KMT2A-N-terminus CUT&RUN in the saturating-occupancy experiment this graph has already designed (exp-residual-occupancy-cutrun-saturating-menin). That experiment currently specifies JUND occupancy gain as its redistribution control; adding ENL costs one antibody and would convert a single patient observation into a testable model.

]- This is the hardest bite-back in the beat: the drug achieved total target engagement — menin undetectable, oncoprotein undetectable — and the disease continued. There is no version of a better menin inhibitor, a covalent menin inhibitor, or even a fusion degrader that answers this observation.
- It attacks the project's central hypothesis at its foundation. The menin-KMT2A interface is a handle on the driver; this mechanism says the driver's *effect* can persist without the driver, which means holding the handle is not the same as controlling the disease.
- It is undetectable by every assay in routine use and by every assay the resistance field is currently investing in. Base-editor screens, MEN1 resequencing and cell-free DNA all read DNA; this is a protein sitting on chromatin.
- The evidence is a single patient sample and the entry could be wrong in either direction — the mechanism could be an artefact of one profile, or it could be the commonest reason menin inhibitors stop working. Both readings are currently permitted by the data, which is itself the finding: nobody has looked twice.
- It contradicts a hypothesis this graph already holds, and the graph should not be left asserting both. The residual-occupancy hypothesis predicts retained fusion; the only human data show the fusion gone and a cofactor retained.
- Does ENL persistence occur in samples that did NOT lineage switch? Until that is answered, the mechanism and the switched state are confounded and the generality is unknown.
- Is ENL retention causal or a passenger? An ENL degrader applied to a menin-inhibitor-resistant model would answer it in weeks, and nobody appears to have run it.
- What holds ENL there once the oncoprotein has gone? ENL's YEATS domain reads acylated histones, so the candidate answer is that the mark outlives the writer — which would make histone acyltransferase inhibition (KAT6/KAT7, KAT2A) the right countermeasure and would explain why that class overcomes non-genetic menin resistance.
- Does the same thing happen with AF9/MLLT3, ENL's paralogue, and with AFF1-SEC components? If cofactor retention is general rather than ENL-specific, the countermeasure has to be the mark, not the reader.
- Would a fusion-degrader rather than an interface inhibitor solve this, or does the lesion already not need the fusion? The revumenib sample says the fusion was already undetectable, which predicts a degrader would not help either — and that is a sharp, falsifiable claim about a whole therapeutic strategy.

MLL oncoprotein levels influence leukemia lineage identities. · Janssens DH, Duran M, Otto DJ, et al. · Nat Commun 2024What this evidence cannot show
ONE sample, inside a 36-sample survey. Abstract read in full via NCBI efetch; full text not obtained, so the number of ENL-retained loci, the antibody and normalisation used, and whether an ENL-negative control sample was profiled are all unknown to this entry. 'Undetectable' is a limit-of-detection statement in primary material. The sample is lineage-switched, and lineage switching is independently associated with reduced oncoprotein levels in the same study, so drug saturation and the switched state are confounded.
Evidence that argues against this (1)
- The alternative model already in this graph predicts the opposite: that a reproducible subset of sites retains FUSION occupancy at saturating drug through CXXC, LEDGF/PSIP1-PWWP and AFF1-SEC tethering. That model is grounded in the demonstration that menin inhibitors act by evicting menin AND MLL1 from chromatin and that resistance mutations work by preventing that eviction — which implies fusion occupancy is the quantity under drug control and should be partially retained, not abolished. - E2moderate confidence 3 of 5: One strong study, or several smaller consistent ones.preprint
The preprint version of the same work states the resistance interpretation directly in its title and conclusion: KMT2A oncoproteins induce epigenetic resistance to targeted therapies, promoting lineage-switching events through dynamic chromatin binding and inducing epigenetic lesions, marked by ENL, that support resistance to targeted therapies.

The preprint of [E1], not an independent confirmation. Cited because the resistance framing is explicit there and was softened to 'lineage identities' in the published title, which is itself worth recording: a reader searching the published literature for menin-inhibitor resistance will not find this paper by its title.
Evidence that argues against this (1)
- E1 and E2 are the same study, preprint and published version, so they are not two independent lines of support and should not be read as corroborating each other. This entry's core claim rests on ONE dataset, and one human sample within it. Blood 2025

Evidence that argues against this (1)
- This result cuts against the entry's own pessimism and is now peer reviewed. KAT6/KAT7 catalytic inhibition rapidly EVICTS the fusion from chromatin and overcomes both primary and acquired menin-inhibitor resistance - so an ENL-marked epigenetic memory, if it exists, is not an immovable end state.
