Hypothesis: releasing the KMT2A::AFF1 differentiation block sends cells toward myeloid, not mature B, in a measurable fraction of infant cases
also called menin inhibitor lineage destination hypothesis · differentiation therapy as lineage-switch driver
Menin inhibitors work by unfreezing leukaemia cells so they can finish maturing. But these particular cells were never fully committed to being B cells, so unfreezing them might turn them into myeloid cells instead of healthy B cells. That would look like a response followed by a different leukaemia.
The same thing, for a scientist
Menin inhibition is a differentiation therapy. In KMT2A::AFF1 infant disease the arrested cell is fetal-type, CD10-negative, pre-VDJ and demonstrably retains latent myeloid potential, and the KMT2A-AFF1 regulatory network shares a core set of nodes between its ALL and AML forms. The hypothesis is that pharmacological release of the block is not lineage-neutral: in a measurable fraction of cases it resolves toward a myeloid program rather than toward mature B-cell maturation, producing a clinical picture indistinguishable from spontaneous lineage switch. If true, menin inhibitor monotherapy in infant t(4;11) is a lineage-switch pressure of the same kind as CD19-directed therapy, and the correct design is a combination or a sequence that closes the myeloid exit before the block is released.
The argument, in four verified steps1 deeper
- The arrested cell has latent myeloid potential. MLL-Af4-transformed human cells can assume a myeloid state under environmental pressure while retaining lymphoid-lineage potential, and the same work identified t(4;11) patients whose leukaemia evaded CD19-directed therapy by myeloid lineage switch 1.
- The regulatory apparatus is pre-wired for both lineages. The KMT2A-AFF1 gene regulatory network contains a core set of nodes present in both the ALL and the AML forms of the disease 2.
- Lineage output is context-determined, not fusion-determined. The same murine Mll-AF4 fusion yields a mixed B-cell-precursor/myeloid leukaemia propagated by LMPPs with one co-driver and a pro-B ALL with another 3.
- Lineage switch is a real clinical event under targeted lymphoid pressure, and it is reversible. An infant KMT2A-rearranged ALL underwent a blinatumomab-induced ALL-to-AML switch and then spontaneously reverted 4; a separate infant B-lymphoblastic leukaemia with t(1;11)(p32;q23) switched lineage following blinatumomab therapy 5.
Step 4 is the one that makes this a hypothesis about drugs rather than about biology. The documented spontaneous reversion 4 tells us the switch is a state transition, not a clonal replacement, in at least some patients. A state that can flip back under no selective pressure is a state that can be flipped forward by a drug that unlocks it.
Why this is not already answered by the trials
Menin inhibitor registration studies are dominated by relapsed/refractory AML and report response by blast count and by differentiation markers, in populations where a myeloid outcome is not an anomaly. The specific question here concerns infant B-lineage t(4;11) disease and requires a readout that trial response criteria do not capture: the lineage identity of the persisting cells at the single-cell level while they are still responding. Differentiation syndrome, which these drugs do cause, is reported as a toxicity and not as a lineage measurement.
Predictions that distinguish this hypothesis from the standard model
| prediction | if hypothesis true | if standard differentiation model true |
|---|---|---|
| single-cell phenotype of persisting cells under menin inhibition in KMT2A::AFF1 | myeloid program activation in a subpopulation, CD19 retained or lost | progressive B-maturation signature, surface IgM appearance |
| chromatin at myeloid promoters after drug | becomes more accessible or loses repressive marks | unchanged or further repressed |
| lineage switch frequency in infant t(4;11) treated with a menin inhibitor | higher than background | equal to background |
| effect of adding myeloid-antigen-directed cover | reduces relapse and switch | no effect on switch, since no switch occurs |
| CD10-negative, IRX-type undifferentiated cases | most likely to switch | no lineage-specific pattern |
The fourth row is the therapeutically decisive one, because it converts the hypothesis into a trial design rather than an observation.
What would kill this hypothesis
This hypothesis is meant to be killable, and the falsifiers are listed in data.falsifiers. The most direct is simple: if serial single-cell profiling of menin-inhibitor-treated KMT2A::AFF1 patients or patient-derived xenografts shows the persisting and responding cells acquiring surface immunoglobulin and a mature B program with no myeloid program activation in any subpopulation, the hypothesis is wrong as stated.
A second, cheaper falsifier: if a systematic review of lineage switch cases shows that switches occur only under antigen-directed pressure (blinatumomab, CD19 CAR-T) and never under chromatin-directed therapy despite comparable exposure, then release of the block is not itself sufficient and antigen pressure is doing the work.
| statement | Pharmacological release of the KMT2A::AFF1 differentiation block, as achieved by menin inhibition, is not lineage-neutral: in a measurable fraction of infant t(4;11) cases it resolves toward a myeloid program rather than toward mature B-cell maturation, making chromatin-directed differentiation therapy a lineage-switch pressure comparable to CD19-directed therapy. |

| origin | literature-gap |

| what it would change | If supported, menin inhibitor monotherapy in infant t(4;11) should not be given without myeloid-directed cover or a defined sequencing strategy, and lineage immunophenotype should become a protocol-mandated pharmacodynamic endpoint rather than a toxicity footnote. |

- The bite-back is the hypothesis: the therapy that works by unlocking differentiation may be the therapy that unlocks the escape route.
- If true, a menin inhibitor could produce an excellent early response by every conventional criterion while selecting the myeloid disease that kills the patient.
- Testing this requires measurements that current trial response criteria do not mandate, so the field could accumulate the failure mode without detecting it.
- Has any menin inhibitor trial collected serial marrow immunophenotype with myeloid markers in B-lineage KMT2A-r patients? If the data already exist unanalysed, this hypothesis could be tested retrospectively for the cost of a query.
- Does the reported spontaneous reversion of a lineage switch imply a bistable regulatory circuit, and if so what is the switch variable? RUNX1, CEBPA and PAX5 are the obvious candidates.
- Is there a menin-independent route to releasing the arrest that is lineage-directed rather than lineage-permissive, for example forcing pre-BCR signalling?

'Environmental pressure' means culture and xenograft conditions rather than a defined molecular signal, and the patient observation is a correlate within a laboratory paper rather than a systematic series.
Evidence that argues against this (1)
- The hypothesis assumes menin inhibition acts on KMT2A::AFF1 ALL as a differentiation therapy, which is the step most likely to be wrong. In primary KMT2A-rearranged samples, ALL showed RAPID, dose-dependent induction of leukemic cell death with revumenib, whereas AML responded much more slowly and by myeloid differentiation. If the lymphoid disease dies rather than differentiates, there is no unfrozen cell to send anywhere and the lineage-destination worry would apply mainly to the myeloid and MPAL forms - which is testable and would substantially narrow the hypothesis. Computationally inferred network with a subset of edges validated; input data are cell-line-heavy, so the shared ALL/AML core may partly reflect shared cell-line artefacts.
Evidence that argues against this (1)
- The gene regulatory network is a reconstruction, not a measurement. It is assembled from ChIP-seq occupancy, bulk patient RNA and CRISPR essentiality in lines, and edges inferred by combining three assays inherit the weakest one: occupancy does not imply regulation, bulk RNA averages over the clusters this beat says the disease splits into, and essentiality comes from cell lines whose lineage composition this project has shown to be confounded. "A core set of nodes shared between the ALL and AML forms" is also the expected result of reconstructing a network from genes both forms express. Identifying factors mediating venetoclax response from the same reconstruction compounds the problem: drug-response edges inferred from a network built on line essentiality are two inferential steps from a patient. The same murine Mll-AF4 fusion produces a mixed B-cell-precursor/myeloid leukaemia propagated by the LMPP population with one microRNA co-driver and a pro-B ALL maintained by an Il7r-positive Kit-positive blast population with another, showing that lineage output is context-determined.

MicroRNA overexpression is a non-physiological perturbation; murine Mll-AF4 models do not fully reproduce human t(4;11) infant ALL.
Evidence that argues against this (1)
- This is murine Mll-Af4 with an enforced microRNA co-driver, and the co-driver is doing the work the conclusion is attributed to. Showing that miR-130b gives an LMPP-propagated mixed leukaemia while miR-128a gives a pro-B ALL demonstrates that TWO DIFFERENT ONCOGENIC COMBINATIONS give two diseases, which is weaker than "lineage output is context-determined": no patient carries either microRNA at those levels. Single case report. Reversion in one patient does not establish that switches are generally reversible, and concurrent chemotherapy could have selected differently rather than the state reverting.
Evidence that argues against this (1)
- A single infant, and spontaneous reversion is precisely the situation in which the alternative explanation — two coexisting clones alternating in dominance as selection pressure is removed — is hardest to exclude. Reversibility of the phenotype at the patient level does not establish reversibility at the cell level. Journal of Clinical Oncology 2025

The arrested KMT2A::AFF1 cell is CD10-negative and pre-VDJ, sitting upstream of the pre-B-cell-receptor checkpoint, so its B-lineage commitment is incomplete at the point where a differentiation therapy would act.

Establishes the arrest position, not the behaviour of that position under a menin inhibitor, which is the untested part of the hypothesis.
Evidence that argues against this (1)
- The premise that the arrested cell is pre-VDJ, and therefore uncommitted at the point a differentiation therapy would act, is contradicted by direct functional mapping. In xenotransplantation of primary infant MLL-rearranged leukaemia, the leukaemia-initiating cells in MLL-AF4 patients were CD34+CD38+CD19+ and CD34-CD19+ committed B cells and generated leukaemia carrying CLONAL IGH rearrangement, while the CD34+CD38-CD19-CD33- fraction from the same patients behaved as normal polyclonal HSCs. On that account the cell a differentiation therapy acts on is already past VDJ recombination and past B-lineage commitment. Aoki Y et al. 2015
