EIF2AK3 / PERK (Q9NZJ5) - the kinase whose transcript is flat under both inhibitors, whose arm IS functionally required in MLL-AF9 leukaemia stem cells, and whose common name collides with phospho-ERK
also called EIF2AK3 · PERK · PEK · Eukaryotic translation initiation factor 2-alpha kinase 3 · E2AK3_HUMAN
PERK is one of four enzymes that can switch on the cell's stress response by tagging a single protein. Its message does not change at all when KMT2A-rearranged cells meet a menin-blocking drug - but a 2024 paper shows that this exact arm keeps MLL-fusion leukaemia stem cells alive, and a PERK-blocking drug reverses it.
The same thing, for a scientist
EIF2AK3/PERK (Q9NZJ5, 1,116 aa, protein-kinase domain 593-1077) is the ER-membrane-resident eIF2alpha kinase and the key effector of the integrated stress response to unfolded proteins. **Its transcript does not move**: RS4;11 array MI-2-2 **+0.32** (DMSO 8.98), VTP-50469 **-0.35** at day 7, EPZ5676 -0.09, MOLM13 +0.08, SEM -0.13, REH +0.09 [E1]. **That is the answer the dispatch brief asked for and it narrows the mechanism**: if the ATF4 output collapse were driven by a transcriptional reconfiguration of the upstream kinases, this is where it would show, and it does not. **But a flat kinase transcript is a bound and not a measurement** - PERK activation is dimerisation and autophosphorylation, entirely post-translational, so these values say nothing about PERK activity. **And the functional literature says the arm matters.** Li 2024 (*Haematologica*) reports that DNAJC10, an HSP40-family member frequently upregulated in AML and in LSC-enriched cells, maintains leukaemia-stem-cell survival and self-renewal, that **its loss specifically induces ER stress and activates the PERK-eIF2alpha-ATF4 branch of the unfolded protein response, that blocking PERK with GSK2606414 or shRNA rescues the DNAJC10 loss-of-function phenotype both in vitro and in vivo, and that Dnajc10 deficiency abrogates AML development and suppresses LSC self-renewal in the MLL-AF9 murine model** [E2]. **This graph could not see that paper either**: `EIF2AK3[tiab]` AND an eight-operand fusion list returns **0**, while `PERK[tiab]` AND the same list returns **5** including it [E3] - the second rule-71 false absence in this beat after PPP1R15A/GADD34. **And PERK is a contaminated token**: `PERK[tiab]` returns **12,317** records against EIF2AK3's 364, sharing 111, and one of the five fusion-scoped hits is a MEK-inhibitor paper in MLL-rearranged infant ALL where **"pERK" means phospho-ERK1/2**, which PubMed indexes identically [E3]. DepMap 26.06: -0.0424 across 1,257 screens, 1.3% below -0.5 [E4].
This is a prediction, not a measurement. No one has seen this protein in this shape. The colours say how much the software trusts itself, and that is worth reading before anything else — the orange stretches are places the model is effectively saying this part has no fixed shape.
I have never read one of these pictures before
A protein is one long chain of amino acids that folds up. The ribbon traces that chain from one end to the other; it is not a wire, it is a simplification of where the backbone runs. Drag to rotate, scroll to zoom.
Domains are the stretches of chain that do a particular job, and they are what the coloured blocks above the picture mark out. Click one and the view flies to it. In this project the important question about any domain is usually whether the fusion still has it — the labels say lost where it does not.
Anything in yellow sticks is a bound small molecule, which is usually the drug.

Well expressed on the array in all three lines (8.98-10.60 log2 RMA), so the null is a measurement. Nothing anywhere reaches 0.4 log2 in either direction, and the array's +0.32 is nominally significant at p 0.030 - another instance of a p-value on a biologically nil change.
What it bounds. There is no transcriptional reconfiguration of the PERK arm under either menin inhibition or DOT1L inhibition in these cells at these timepoints. Taken with EIF2AK1 (+0.26 max), EIF2AK4 (+0.12 max) and EIF2S1 (-0.31 max), the entire eIF2alpha kinase/substrate module is transcriptionally silent while its output collapses 1.4 to 5.1 log2 1. For a graph that has been describing this as "loss of the integrated stress response", that is the specific narrowing the dispatch brief asked for.
What it emphatically does NOT bound. PERK is activated by oligomerisation and trans-autophosphorylation in response to ER client load; its mRNA level is not the regulated variable and never was. A flat PERK transcript is compatible with PERK activity having gone up, down or nowhere, and this entry makes no claim about it. The measurement that would is phospho-PERK or phospho-eIF2alpha, and neither exists under a menin inhibitor in any cell.
The PERK arm IS functionally required in an MLL-fusion model, and a drug exists
Li 2024, Haematologica, PMID 37496439, DOI 10.3324/haematol.2023.282691 2. From the abstract:
- DNAJC10, an HSP40-family member, is frequently upregulated in various types of AML and in LSC-enriched cells.
- DNAJC10 deficiency causes a dramatic increase in apoptosis in human leukaemia cell lines and LSC-enriched populations, and is not required for normal haematopoiesis.
- "deficiency of Dnajc10 significantly abrogated AML development and suppressed self-renewal of LSC in the MLL-AF9-induced murine leukemia model".
- Mechanism: "inhibition of DNAJC10 specifically induces endoplasmic reticulum stress and promotes activation of PERK-EIF2alpha-ATF4 branch of unfolded protein response".
- "Blocking PERK by GSK2606414 (PERKi) or shRNA rescued the loss of function of DNAJC10 both in vitro and in vivo."
- DNAJC10 deficiency increases AML sensitivity to daunorubicin and cytarabine.
Why this is the most important thing in this entry and why it points the opposite way to the menin-inhibitor data. Here the PERK-eIF2alpha-ATF4 branch, when activated, is what kills the leukaemia stem cell - and blocking PERK rescues it. In this beat's data a menin inhibitor lowers ATF4 output 1.4 to 5.1 log2. If both hold in the same cells, a menin inhibitor is doing to the PERK-eIF2alpha-ATF4 axis what GSK2606414 does - and GSK2606414 rescues leukaemia stem cells. That is a specific, falsifiable, uncomfortable prediction: menin inhibition might protect the LSC compartment on this axis while killing the bulk population.
Four reasons for restraint, all of them real. (a) The model is murine MLL-AF9, myeloid, not the index B-lineage KMT2A::AFF1 (rule 73). (b) The axis is engaged there by removing a chaperone, not by a menin inhibitor, and nothing links the two perturbations. (c) This beat measured mRNA, and the rescue experiment is about kinase activity. (d) Abstract only - the figures were not obtained. So this is written as a hypothesis with a named falsifier, not as a finding: measure phospho-eIF2alpha and ATF4 protein in RS4;11 and in a KMT2A-r PDX under VTP-50469, and score the stem-cell-enriched fraction separately from the bulk.
`PERK` and `pERK` are the same token to PubMed
Rule 88: no EIF2AK3 or PERK row exists in data/symbol-collisions.json (157 rows read 2026-09-22). New measurement, and it is a first-class trap 3:
| token | records | shared with EIF2AK3 | Jaccard |
|---|---|---|---|
EIF2AK3[tiab] | 364 | - | - |
PERK[tiab] | 12,317 | 111 | 0.0108 |
Thirty-four times as many records under the common name, sharing 0.9% of the union. Unlike CHOP - where the extra records were a chemotherapy regimen and none was about the gene - most of the 12,317 ARE genuine PERK papers, because the field almost never writes EIF2AK3. So the contamination and the coverage run together, and the fusion-scoped result shows both halves at once:
| query | n | the records |
|---|---|---|
EIF2AK3[tiab] AND the eight fusion operands | 0 | a well-formed, FALSE absence |
PERK[tiab] AND the same operands | 5 | 37496439 (DNAJC10/PERK in MLL-AF9 LSC - the real hit), 37802982 (calcium-sensing receptor in leukaemia), 40640087 (MLL1 in cochlear hair cells), 23025505 (CBL mutations in paediatric AML), 27588400 ("MEK inhibition is a promising therapeutic strategy for MLL-rearranged infant acute lymphoblastic leukemia") |
PMID 27588400 is the specimen. A MEK-inhibitor paper in MLL-rearranged infant ALL - squarely this project's disease - matches a PERK[tiab] query because "pERK" (phospho-ERK1/2) and "PERK" are the same string to a case-insensitive index. Phospho-ERK is ubiquitous vocabulary in leukaemia signalling papers, so any PERK query in this project is contaminated by exactly the papers most likely to look relevant.
And the two halves of the rule pull in opposite directions, which is why the row matters: you must widen EIF2AK3 to PERK or you will miss the only functional paper; and you must title-check every PERK hit or you will import phospho-ERK papers about your own disease.

| localization | ENDOPLASMIC RETICULUM MEMBRANE - AND FOR THE HUMAN PROTEIN THAT CALL IS ECO:0000250, BY SIMILARITY TO MOUSE Q9Z2B5, NOT A HUMAN EXPERIMENT. UniProt Q9NZJ5's location block carries no ECO:0000269 at all, with an uncoded note that PERK is also present at mitochondria-ER contact sites. So even the compartment of the textbook ER stress kinase is, in the human record, an inference. Recorded as uncoded/inferred per rule 91. No PERK localisation experiment exists in any haematopoietic or KMT2A-rearranged cell. |
| subnuclear localization | NOT APPLICABLE AND NOT ASKABLE. PERK is an ER-transmembrane protein with no annotated nuclear pool, coded or uncoded. There is no subnuclear question and none is invented. Recorded as a bounded absence resting on a prior absence. |
| cell type | RS4;11 and SEM (t(4;11) KMT2A::AFF1), REH (KMT2A wild type) and MOLM13 (KMT2A::MLLT3 AML) - flat in all four [E1]. The functional work is in human AML lines, LSC-enriched populations, and murine MLL-AF9 leukaemia [E2]. UniProt records ubiquitous expression with high levels in secretory tissues. No primary KMT2A::AFF1 sample and no normal human haematopoietic cell. |
| cell state | The transcript was measured in the day-6/day-7 drug-arrested state and is flat there. THE STATE THAT MATTERS FOR THIS GENE IS THE STEM-CELL STATE, and it is the one this beat cannot address: the functional PERK result is specifically about LSC survival and self-renewal in LSC-enriched populations and in serial transplantation, and bulk cell lines contain no stem-cell compartment. PERK's activating input is ER client load, which neither public experiment measured or controlled. |
| epigenetic readout | No writer, eraser or reader activity - PERK is an ER-membrane kinase. Its epigenetic relevance here is entirely indirect: its downstream output (ATF4 target genes) falls identically under a menin inhibitor and a DOT1L inhibitor, which places the OUTPUT on the H3K79-methylation-coupled side of the fusion programme while the kinase itself is transcriptionally untouched. No histone-mark measurement at the EIF2AK3 locus exists in any cell. |
| shown for fusion | TRANSCRIPT MEASURED AND FLAT in KMT2A::AFF1 (RS4;11 and SEM) and KMT2A::MLLT3 (MOLM13), with a KMT2A-wild-type control. A FUNCTIONAL requirement for the PERK-eIF2alpha-ATF4 branch is shown in murine MLL-AF9 leukaemia stem cells, where blocking PERK RESCUES a lethal DNAJC10-loss phenotype (PMID 37496439) - so the arm is engaged in an MLL-fusion model, in the wrong fusion, the wrong lineage and the wrong species. Nothing at the protein or phospho level in any KMT2A::AFF1 cell. |

}- A PERK inhibitor RESCUES leukaemia stem cells in an MLL-AF9 model. A menin inhibitor lowers the output of the same PERK-eIF2alpha-ATF4 axis. If those two facts compose, menin inhibition is protecting the stem-cell compartment on this axis while killing the bulk population - which is exactly the shape of a relapse mechanism, and nothing in a bulk transcriptome or a MEN1 sequencing panel would reveal it.
- Any therapeutic attempt to ACTIVATE this arm (an ER-stress inducer, a chaperone inhibitor, a proteasome inhibitor) would be working against the menin inhibitor's measured effect on the same axis, and vice versa. The two strategies are candidates for mutual antagonism and neither has been tested against the other.
- In this project specifically, searching this gene by its common name imports papers about MEK inhibition in MLL-rearranged infant ALL, because pERK and PERK are one token. Any bounded absence, coverage count or citation audit in this graph that used PERK as a search string is contaminated by the most relevant-looking possible false positives.
- Is phospho-eIF2alpha changed in RS4;11 under VTP-50469? Every mechanistic claim in this beat's ATF4 story turns on it, no menin-inhibitor study in any cell has measured it, and it is one lane on a blot.
- Does menin inhibition protect the leukaemia-stem-cell compartment on this axis? A PERK inhibitor rescues LSC self-renewal in MLL-AF9 mice, a menin inhibitor lowers ATF4 output, and nobody has scored the stem-cell-enriched fraction separately from the bulk under a menin inhibitor.
- Is DNAJC10 expressed and does it change in KMT2A::AFF1 cells? It was not in this beat's pre-registered panel, it is reportedly upregulated in AML and LSC-enriched cells, and it is the chaperone whose loss engages this arm.
- Does GSK2606414 or a modern PERK inhibitor antagonise VTP-50469 in RS4;11? The prediction from Li 2024 is that it should, if the menin inhibitor is already acting on the same axis - and antagonism would be the clean demonstration that the ISR arm is doing something the drug does not want.

What this evidence cannot show
THE BOUND IS TRANSCRIPTIONAL ONLY, and this is the entry's central caveat rather than a footnote: PERK is activated by oligomerisation and trans-autophosphorylation in response to ER client load, so its mRNA is not the regulated variable and a flat transcript is compatible with PERK ACTIVITY having risen, fallen or not moved. No claim about activity is made. The array probe is well expressed in all three lines (8.98-10.60 log2 RMA) so the null is a measurement and not a detection failure; the second probe (241463_at, 3.33) is at background and is reported as background. The +0.32 at p 0.030 is another nominally significant, biologically nil change. RS4;11 array DMSO arm n=2.
Evidence that argues against this (1)
- Reading a flat kinase transcript as narrowing the mechanism assumes the ATF4-output collapse would have to be driven transcriptionally from the kinase layer if it were driven from there at all. It need not be: PERK could be inactivated by a change in ER client load, by a phosphatase, or by the fusion protein binding a feedback component - and this same beat found that KMT2A fusion proteins were reported in 1999 to bind GADD34, the eIF2alpha-phosphatase adaptor. The flat transcript is consistent with a fully reconfigured pathway. Adler HT et al. 1999

- This result points OPPOSITE to the menin-inhibitor observation: here ACTIVATION of PERK-eIF2alpha-ATF4 is what kills the leukaemia stem cell and blocking PERK rescues it, while a menin inhibitor LOWERS ATF4 output 1.4 to 5.1 log2. If both hold in the same cells, a menin inhibitor is doing on this axis what a PERK inhibitor does - which would protect the LSC compartment while killing the bulk population. That is an uncomfortable prediction and it is unfalsified because nobody has scored the stem-cell fraction separately under a menin inhibitor. NCBI GEO, re-analysed here 2026

All five PERK-scoped titles were fetched and read; none was assumed. Rule 3 was satisfied on the EIF2AK3 zero AND THE ZERO WAS STILL FALSE - the second instance of that in this beat after PPP1R15A/GADD34. Note the two halves of this trap pull opposite ways: you MUST widen to PERK to find the functional paper, and you MUST title-check every PERK hit because phospho-ERK contaminates exactly the papers most likely to look relevant in a leukaemia project. Rule 94 form; rule 95 observed; rule 80: titles and abstracts only. NOT in data/symbol-collisions.json.
Evidence that argues against this (1)
- Calling PERK 'contaminated' is only half true and the distinction matters: unlike CHOP, where none of the twelve extra records was about the gene, MOST of PERK's 12,317 records ARE genuine PERK papers, because the field almost never writes EIF2AK3. So the canonical symbol is the deficient instrument here and the alias is the better one, subject to title-checking - the opposite prescription from the DDIT3/CHOP case in this same beat. Recorded as uncoded/inferred rather than as a measurement. The fifteen PDB entries were NOT fetched. DepMap figures from the same session with the release read back (rule 83).
Evidence that argues against this (1)
- A -0.04 DepMap gene effect with 1.3% of lines below -0.5 means PERK is dispensable in essentially every cancer cell line screened, which is difficult to reconcile with a paper reporting that PERK inhibition rescues a lethal phenotype - rescue requires the axis to be doing something. The resolution is that the screens never apply ER stress, but it does mean there is no independent functional-genomics support for PERK mattering in leukaemia. DepMap / Open Targets Platform 2026
PERK phosphorylates eIF2alpha and is the key effector of the ISR to unfolded proteins, raising ATF4 translationally; its own transcript is flat under both inhibitors (max 0.35 log2) while ATF4 output falls 1.4-5.1

the PERK-eIF2alpha-ATF4 branch is functionally required in MLL-AF9 murine leukaemia stem cells: DNAJC10 loss activates it and blocking PERK with GSK2606414 RESCUES the phenotype (PMID 37496439)

the state that matters for this kinase is the leukaemia-stem-cell state, which bulk cell lines cannot address; a PERK inhibitor rescues LSC self-renewal in MLL-AF9 mice
PERK's transcript is flat in all four lines on a well-expressed probe, so the null is a measurement and not a detection failure
