CDK6 - the kinase the KMT2A-r rationale actually rests on, and the pan-essentiality flag and the selectivity are both true

also called CDKN6 · PLSTIRE · cell division protein kinase 6

CDK6 is one of two closely related enzymes that let a cell commit to dividing, and it is the one KMT2A-fusion leukemia cells actually need - its twin, CDK4, they barely need at all. But normal blood stem cells need CDK6 too, so a drug that blocks it is aiming at a difference in degree, not a switch that is on in the cancer and off everywhere else.

The same thing, for a scientist

CDK6 is the G1 cyclin-dependent kinase that carries the entire CDK4/6 rationale in KMT2A-rearranged leukaemia. Two independent groups showed CDK6-versus-CDK4 discrimination by knockdown in this disease, and this project's genome-wide DepMap scan reproduces it unbiasedly: CDK6 ranks 13 of 17,728 for KMT2A-rearranged selectivity while CDK4 ranks 16,052 [E3]. The coverage ledger flags CDK6 as pan-essential, and by the project's own threshold (mean Chronos effect across all lines at or below -0.5) that flag is correct: CDK6's mean is -0.6321 across 1,258 lines, with 51.9% of lines at or below -0.5 [E4]. The flag and the selectivity are not in conflict, and reconciling them is the point of this entry. Split by lineage, CDK6's mean effect is -1.3767 in 57 leukaemia lines against -0.4464 for CDK4, while across all lines CDK4 is the deeper dependency (-0.7371) [E4]. CDK6's breadth is real; its depth is haematopoietic. Three things then bound the window: normal murine HSCs require CDK6 for competitive repopulation and 5-FU recovery [E5], part of CDK6's leukaemic function is kinase-independent and therefore invisible to an ATP-competitive inhibitor [E6], and no approved agent is CDK6-selective.

This is a prediction, not a measurement. No one has seen this protein in this shape. The colours say how much the software trusts itself, and that is worth reading before anything else — the orange stretches are places the model is effectively saying this part has no fixed shape.

I have never read one of these pictures before

A protein is one long chain of amino acids that folds up. The ribbon traces that chain from one end to the other; it is not a wire, it is a simplification of where the backbone runs. Drag to rotate, scroll to zoom.

Domains are the stretches of chain that do a particular job, and they are what the coloured blocks above the picture mark out. Click one and the view flies to it. In this project the important question about any domain is usually whether the fusion still has it — the labels say lost where it does not.

Anything in yellow sticks is a bound small molecule, which is usually the drug.

The ledger asked a question; here is the number that answers it

docs/coverage.md priority 7 says: CDK6, not CDK4/6 - any entry recommending the class should say which gene carries the rationale, and adds that CDK6 comes back pan-essential in the unwritten-interactor triage, which is an argument about the therapeutic window the priority as written does not make.

Both halves are right and they do not cancel. Fetched live from DepMap (Chronos) through the Open Targets GraphQL API on 2026-09-20 4:

fraction of lines at or below -1.023.1%22.3%
mean, 57 leukaemia lines-1.3767-0.4464

Three readings follow, and only the third is new.

  1. The pan-essentiality flag is arithmetically correct. CDK6 at -0.63 clears the project's -0.5 threshold, and half of all cancer lines lose fitness on CDK6 knockout. A reader told CDK6 is selectively required in KMT2A-r, and not told this, has been misled.
  2. CDK4 is the more pan-essential of the two (-0.7371 against -0.6321; 58.4% against 51.9% of lines below -0.5) - which is not what the phrase CDK4/6 inhibitor, and CDK6 is the pan-essential one would lead anyone to expect.
  3. The two facts are reconciled entirely by lineage. The kinases partition by tissue: CDK6 is the G1 kinase of haematopoietic cells, CDK4 of most solid tissues. Pooling 1,258 lines averages across that partition and hides it. In leukaemia the separation is 0.93 gene-effect units, in the direction the literature predicts.

So the window is real and it is a lineage window, not an oncogene window. That is a materially weaker claim than KMT2A-fusion cells are addicted to CDK6, and it is the claim the data support.

Which is exactly the problem, because the lineage is the dose-limiting toxicity

The window is widest in the tissue whose normal counterpart is what a CDK4/6 inhibitor is already known to damage. Neutropenia is the dose-limiting toxicity of every approved agent in this class, and the single dose-limiting toxicity in the COG paediatric palbociclib trial was haematologic (see target-cdk4-6-inhibition-kmt2a-r).

The mouse genetics say the same thing directly: in murine BCR-ABL disease, Cdk6-null HSCs fail to repopulate competitively and Cdk6-deficient mice are significantly more susceptible to 5-fluorouracil 5. CDK6 there is not acting as a cell-cycle kinase but as part of a transcriptional complex suppressing Egr1.

A selective-CDK6 argument is therefore not straightforwardly better than a CDK4/6 one. It is a trade: remove the CDK4 arm, which is the broader dependency across tissues generally and the one this disease does not preferentially need - and concentrate the remaining pressure on the haematopoietic compartment, in an infant, during marrow failure. The honest statement is that CDK6-selectivity improves the specificity of the rationale and does not obviously improve the safety margin in marrow.

And a knockout is not an inhibitor here, for a reason specific to this kinase

Every number above is CRISPR knockout. For most targets, treating knockout as a proxy for inhibition is a generic caveat. For CDK6 it is a specific, published defect.

In a murine p185 BCR-ABL+ B-lymphoid leukaemia model, CDK6 is part of a transcription complex that induces p16INK4a and VEGF-A, and this function is independent of CDK6's kinase activity 6. The same group's HSC work puts the Egr1 suppression in the same non-catalytic register 5. An ATP-competitive inhibitor removes the kinase arm and leaves the scaffold; a CRISPR knockout removes both.

So the chain CDK6 ranks 13th on selectivity, therefore a CDK4/6 inhibitor should work in KMT2A-r has a real break in it, and the break is not hypothetical. It is the same break the cyclin literature flags from the CCND3 side 7.

Where CDK6 sits relative to the fusion, and to the rest of this beat

CDK6 is a transcriptional output of the fusion, not a component of it. In MLL-rearranged infant ALL and human MLL-rearranged lines, CDK6 mRNA is higher than in wild-type infant ALL, falls on siRNA knockdown of MLL-AF4 or MLL-ENL, and CDK6 - but not CDK4 - knockdown inhibits growth of the KMT2A::AFF1 line SEM 1. In murine MLL-AF9 AML, cells are exceptionally reliant on CDK6 but not CDK4, CDK6 is transcriptionally activated by mutant MLL, and the growth effect runs through myeloid differentiation 2.

That places CDK6 at the far end of the axis this beat is about: BMI1 represses CDKN2A, CDKN2A encodes p16INK4a, and p16INK4a is the endogenous inhibitor of CDK4 and CDK6. The three are one circuit (mechanism-bmi1-cdkn2a-cdk6-axis), and CDK6 closes it from the other direction by inducing p16INK4a itself 6. In infant KMT2A-r ALL CDKN2A is deleted in only about 8.5% of cases (gene-cdkn2a), so in most patients the brake is present and held down rather than removed - which is the situation in which a polycomb intervention and a CDK6 intervention are alternatives aimed at one node rather than an orthogonal pair.

therapeutic window verdictREAL BUT LINEAGE-SHAPED, AND NARROWER THAN THE LEDGER PRIORITY IMPLIES. CDK6 is genuinely the gene that carries the KMT2A-r rationale and CDK4 genuinely does not. But CDK6 is not selectively required BY THE FUSION so much as selectively required BY HAEMATOPOIETIC CELLS, and the normal haematopoietic compartment is the dose-limiting tissue. A CDK6-selective agent would sharpen the rationale and would not obviously widen the marrow margin.

How this could bite us back
  • The window is a lineage window, and the lineage is the toxicity. CDK6 dependency is deepest in haematopoietic cells; neutropenia is the dose-limiting toxicity of this class; the one DLT in the COG paediatric palbociclib trial was haematologic. Going CDK6-selective concentrates the pressure where the marrow already is.
  • Cdk6-null HSCs fail competitive repopulation and Cdk6-null mice are more sensitive to 5-fluorouracil [E5]. That is an essential-function toxicity in exactly the compartment under stress during induction, and it is invisible in steady state - so a tolerability signal in an unstressed patient does not bound it.
  • Part of CDK6's leukaemic function is kinase-independent [E6]. An ATP-competitive inhibitor removes the catalytic arm and leaves the scaffold, so the DepMap knockout number is an upper bound on what any current drug can deliver, not a prediction of it.
  • CDK6 induces p16INK4a [E6]. Inhibiting CDK6 in a cell where CDKN2A is intact - which is about 91% of infant KMT2A-r ALL - may relieve the very brake the therapy depends on, and the direction of that feedback has never been measured in this disease.
  • Cytostatic, not cytotoxic. A G1 arrest leaves cells alive in a disease whose hazard is front-loaded, and an arrested clone is a clone available to acquire resistance.
  • RB1 loss is the canonical resistance route for this class, and live DepMap gives RB1 a POSITIVE mean gene effect (+0.1801 across 1,258 lines, +0.1127 in leukaemia lines) - losing RB1 is fitness-neutral to favourable, so nothing stops a treated clone from taking that exit.
  • The target is a fusion output. Any escape that lowers dependence on the fusion programme - including the documented lineage switch - takes the dependency with it.
  • the inhibitor targets a pool that may not be the relevant one - Only nuclear CDK6 complexes are catalytically active and they are a minority of cellular CDK6; the transcriptional function at promoters is partly kinase-independent. An ATP-site CDK4/6 inhibitor and a CDK6 degrader are therefore not interchangeable in KMT2A-rearranged disease, and nobody has measured the CDK6 partition or chromatin occupancy in a KMT2A-rearranged cell to say which matters.
  • paralogue escape - CDK4, not CDK6, is absent from the FLT3/PIM1 promoters in AML, so a pan-CDK4/6 inhibitor perturbs two proteins with different chromatin behaviour and a resistance route through CDK4 up-regulation would look like on-target failure.
Still open
  1. Does CDK6's kinase-independent transcriptional function operate in KMT2A-rearranged cells at all? Every published demonstration is in BCR-ABL+ or normal HSC systems. A kinase-dead CDK6 knock-in rescue in a KMT2A::AFF1 line would settle whether a CDK4/6 inhibitor can phenocopy the knockout this graph's rank-13 claim is built on.
  2. ANSWERED IN PART, and the answer is the opposite of the worry: menin-inhibitor treatment ATTENUATES CDK6 levels in MLL1-rearranged AML, and yet the menin-inhibitor plus abemaciclib combination is SYNERGISTIC rather than redundant [E8]. What remains open is whether this holds in B-lineage KMT2A::AFF1, where the differentiation mechanism that both agents are said to act through has no myeloid counterpart.
  3. Is the leukaemia-lineage CDK6 dependency present in B-lineage lines specifically, or is the 57-line mean carried by myeloid lines? That split was not computed this cycle and the index arm of the project's scan is myeloid-enriched.
  4. What is the CDK6 gene effect in an infant-derived KMT2A::AFF1 line? None exists in DepMap; ALL-PO (CVCL_1069, age 4 months) is in the collection with no CRISPR value, which is already ledger priority 16.
  5. Is the menin-inhibitor/CDK4-6 synergy CDK6-specific or generic? In NUP98-rearranged leukaemia, palbociclib and gilteritinib - two unrelated kinase inhibitors - converged on the same pathways when combined with a menin inhibitor [E8 counter-evidence], which suggests the synergy may be a proliferation-plus-differentiation effect available to many partners.
  6. What is the nuclear/cytoplasmic partition of CDK6 in KMT2A-rearranged blasts, and is CDK6 on chromatin at HOXA9, MEIS1 or FLT3 there? If the relevant pool is the chromatin-bound transcriptional one, a degrader and an ATP-site inhibitor would separate cleanly in this disease and nobody has tested it.

The leukaemia split is by the screen's own diseaseFromSource string matched on leukemia/leukaemia/lymphoblastic/myeloid, which is a coarse text match and not a curated lineage annotation, and it is not conditioned on fusion status. The DepMap release version is not stated by the API. In a murine p185 BCR-ABL+ B-acute lymphoid leukaemia model, CDK6 is part of a transcription complex that induces expression of the tumour suppressor p16INK4a and of VEGF-A, and this function is independent of CDK6's kinase activity; high CDK6 therefore suppresses proliferation by upregulating p16INK4a unless p16INK4a is absent, in which case CDK6 exerts its full tumour-promoting function.

A kinase-independent function of CDK6 links the cell cycle to tumor angiogenesis · Kollmann K, Heller G, Schneckenleithner C, Warsch W, Scheicher R, Ott RG, et al. · Cancer Cell 24:167-181 2013
    What this evidence cannot show

Abstract read this cycle. Murine BCR-ABL+ B-ALL, not KMT2A-rearranged, and the angiogenesis half of the claim is not obviously transferable to a marrow-resident leukaemia. The load-bearing part for this entry is narrower and better supported: a documented non-catalytic CDK6 function in a lymphoid leukaemia, which is enough to break the knockout-to-inhibitor inference without the VEGF biology having to hold here.

Evidence that argues against this (1)
    • Against reading kinase-independence as the whole story: two independent groups report that small-molecule CDK6 or CDK4/6 inhibition phenocopies CDK6 depletion in MLL-rearranged cells, which argues the catalytic arm does carry real weight in this specific context. Placke T et al. 2014

CCND3 is indispensable for the maintenance of B-cell acute lymphoblastic leukaemia independent of mutational background, and its anti-apoptotic effect is independent of the kinase activity of the CCND3-CDK4/6 complex, leading the authors to conclude that downregulating CCND3 might be superior to inhibiting CDK4/6 kinase activity.

Abstract read this cycle. Cited here only for the kinase-independence point, which converges with E6 from the cyclin side. B-ALL generally, not selected for KMT2A rearrangement. One paper, not independently replicated in anything read here.

Evidence that argues against this (1)
    • Live DepMap shows CCND3 is a much deeper dependency in leukaemia lines (mean -1.0076 across 57) than across all lines (-0.3459), so the cyclin and the kinase track each other by lineage - which is also what a genuinely kinase-dependent complex would look like. In human AML cell lines and patient-derived AML cells harbouring MLL1 rearrangement or mutant NPM1, menin-inhibitor treatment ATTENUATES CDK6 levels, and co-treatment with the menin inhibitor SNDX-50469 and the CDK4/6 inhibitor abemaciclib induces SYNERGISTIC lethality; the same study reports synergy with venetoclax and shows the combinations are preserved against MLL1-r AML cells expressing mutant FLT3 and CRISPR-edited to introduce mutant TP53.

    What this evidence cannot show

Abstract read this cycle; full text not retrieved. The system is AML - human AML cell lines and patient-derived AML cells with MLL1 rearrangement or mutant NPM1 - NOT infant B-lineage KMT2A::AFF1, and the in vivo arm of the paper is the venetoclax combination, not the abemaciclib one, so the CDK4/6 synergy is reported in vitro and ex vivo only. No synergy statistic, concentration or line identity was read. Abemaciclib is a dual CDK4/6 agent, so the synergy cannot be attributed to CDK6 alone.

Evidence that argues against this (1)
    • The parallel result in NUP98-rearranged leukaemia found that palbociclib and gilteritinib, despite inhibiting two unrelated kinases, affected SIMILAR pathways when combined with a menin inhibitor - cell cycle, proliferation and differentiation - which is an argument that the synergy is a generic proliferation-plus-differentiation effect rather than a CDK6-specific one. In human T cells CDK6 partitions across at least three complexes with different compartments - the abundant 450 kDa Hsp90/CDC37 complex is predominantly cytoplasmic while the 170 kDa cyclin D/CDK6 complex is in both - and only the NUCLEAR CDK6 complexes have kinase activity, so catalytically active CDK6 is a minority of total CDK6.

    What this evidence cannot show

1998 biochemistry in normal T cells, not leukaemia and not a KMT2A-rearranged cell. Fractionation leaks between compartments, and the study predates the kinase-independent transcriptional functions later attributed to CDK6, which a kinase-activity readout scores as zero. Abstract only.

Evidence that argues against this (1)
    • CDK6 has documented kinase-INDEPENDENT transcriptional functions, so 'only nuclear CDK6 is active' is true of catalytic activity and may be false of function; a cytoplasmic pool scored inactive by kinase assay is not thereby inert. Uras IZ et al. 2016

In FLT3-ITD acute myeloid leukaemia cells CDK6, but not its functional homolog CDK4, is found at the promoters of the FLT3 and PIM1 genes where it regulates their transcription, and palbociclib kills these cells through that transcriptional activity.

    What this evidence cannot show

FLT3-ITD AML, not KMT2A-rearranged, and cell-line rather than patient material. ChIP is a genomic address, not a subnuclear compartment. The CDK4-versus-CDK6 discrimination is the load-bearing detail and is a paralogue distinction of exactly the class this graph's registry warns about. Abstract only.

Evidence that argues against this (1)
    • CDK6 is a recurrent KMT2A-fusion target gene and the CDK4/6-inhibitor rationale in this disease rests on the cell-cycle kinase function; if the relevant pool in KMT2A-rearranged cells were the chromatin-bound transcriptional one, an ATP-site inhibitor and a degrader would not be interchangeable - and nobody has measured the partition here. Elevated NUCLEAR CDK6 specifically, scored by compartment on immunohistochemistry, is associated with unfavourable prognosis in lung adenocarcinoma patients - evidence that the nuclear/cytoplasmic partition of CDK6 is discriminating in tissue and not only in fractionation.

Graded weak: a small single-cohort IHC association in a solid tumour, in a low-visibility journal, and the PubMed record carries NO DOI (PMID only, PMC6965964). Compartment scoring by IHC depends on the antibody and on fixation. Cited for the existence of a compartment-resolved readout, not for the prognostic number. Abstract only.

Evidence that argues against this (1)
    • An IHC compartment score cannot distinguish the kinase-active cyclin D/CDK6 complex from the Hsp90/CDC37-bound pool, which the biochemistry shows are differently distributed; a nuclear signal is not by itself an active-CDK6 signal. Mahony D et al. 1998
