CALM-AF10 strips H3K79 methylation from the rest of the genome while adding it at HOXA - one fusion, two opposite effects on the same mark
also called CALM-AF10 global H3K79 hypomethylation · DOT1L sequestration
The CALM-AF10 fusion grabs the DOT1L enzyme and takes it away from most of the genome, so the chemical tag DOT1L writes almost disappears everywhere except at a few growth genes. Cells with less of that tag break their chromosomes more easily.
The same thing, for a scientist
The commonest statement about this fusion - 'CALM-AF10 drives H3K79 methylation' - is **globally false and focally true**. In human and murine CALM-AF10-positive leukaemic cells the fusion **greatly reduces global H3K79 methylation** by competing with endogenous AF10 for DOT1L in a dominant-negative fashion, disrupting AF10-mediated association of DOT1L with chromatin [E1]. Hypomethylated cells are more sensitive to gamma-irradiation and show increased chromosomal instability, and CALM-AF10 patients carry more secondary chromosomal aberrations than patients with other fusion-driven leukaemias [E1]. The OM-LZ is required for the hypomethylation, so it is the same interface that produces the focal gain [E1]. A second, non-exclusive mechanism is **cytoplasmic mislocalisation**: in murine embryonic fibroblasts stably expressing CALM-AF10, about half the cells show H3K79 hypomethylation together with cytoplasmic colocalisation of CALM-AF10 and DOT1L, and mutating the CALM NES rescues the hypomethylation and retains both proteins in the nucleus [E2]. **The two accounts - competition on chromatin versus physical export out of the nucleus - are not the same claim and the field has not chosen between them.**
Why it matters beyond bookkeeping
- It inverts the intuition behind DOT1L inhibition here. In KMT2A-rearranged leukaemia the rationale is 'too much H3K79 methylation at fusion targets, so remove it'. In CALM-AF10 the genome already has far too little, and the therapeutic target is a small focal excess sitting inside a global deficit. A DOT1L inhibitor deepens the global lesion while removing the focal one 3.
- It supplies a cooperating-mutation mechanism. The long latency of CALM-AF10 transgenic mice implies cooperating events; genome instability from global H3K79 loss is a mechanism for generating them 1. The observed co-mutation spectrum - TP53 and NF1 in myeloid cases, EZH2 in T-lineage, PHF6 shared - is consistent with a mutator-permissive background, though nothing in this beat's reading demonstrates causality.
- It is a measurable pharmacodynamic trap. Total cellular H3K79me2 in a CALM-AF10 cell is dominated by whatever endogenous signal survives, so a bulk western is a poor readout of anything the fusion is doing. Locus-resolved, calibrated ChIP is required.

- A DOT1L inhibitor in this disease removes a focal excess sitting inside a global deficit, and the global deficit is what drives chromosomal instability. Deepening it in a cell that already breaks chromosomes is a hypothesis nobody has tested.
- Genome instability is also a resistance engine: a fusion that generates secondary aberrations at elevated rate will generate resistance alleles at elevated rate against any targeted agent.
- Competition on chromatin or physical export? A DOT1L tethering experiment that forces nuclear retention without altering the fusion would separate them.
- Does global H3K79 hypomethylation actually cause the cooperating mutations, or merely accompany them? The co-mutation spectra are known; the causal experiment is not.
- Is the same global depletion present in KMT2A::MLLT10, which carries the same OM-LZ but no NES? If not, the NES route is the explanation; if so, competition is.

In human and murine CALM-AF10-positive leukaemic cells, the fusion greatly reduces global H3K79 methylation by disrupting AF10-mediated association of hDOT1L with chromatin; cells with reduced H3K79 methylation are more sensitive to gamma-irradiation and show increased chromosomal instability, and patients with CALM-AF10 carry more secondary chromosomal aberrations.

Mixed human leukaemic cells and murine models. The patient-level secondary-aberration comparison is observational and CALM-AF10 cases are few. The paper is registered as NCT00266136, which is a clinical study registration rather than a trial of an intervention tested here.

- A rival explanation for the same global phenotype is physical mislocalisation of DOT1L to the cytoplasm rather than competition on chromatin; mutating the CALM NES rescues hypomethylation and keeps both proteins nuclear, which a competition model does not obviously predict. Conway AE et al. 2013

In murine embryonic fibroblasts stably transduced with PICALM::MLLT10, about half of cells display H3K79 hypomethylation with cytoplasmic colocalisation of CALM-AF10 and DOT1L, and mutating the CALM NES rescues the hypomethylation while retaining both proteins in the nucleus.

Murine fibroblasts with stable overexpression - not a haematopoietic cell and not an endogenous expression level. 'About half of cells' is a penetrance figure, so the phenotype is not uniform even in a clonal population.

- PUBLISHED CONFLICTING RESULT that removes this item's mechanism while leaving its phenomenon. The account here is cytoplasmic sequestration: the fusion drags DOT1L out of the nucleus, global H3K79 falls, and mutating the NES rescues by retaining both proteins in the nucleus. In leukaemia cells that account fails at the first step - full-length CALM-AF10 LOCALISED TO THE NUCLEUS and STILL SUPPRESSED H3K79 METHYLATION, regardless of the presence of clathrin. A nuclear fusion that lowers global H3K79 needs a different explanation: competition for DOT1L on chromatin, redistribution of the enzyme between loci, or an effect on the H3K79me1-to-me2 conversion. The murine fibroblast result stands as a measurement; the sequestration interpretation should not be carried into a haematopoietic context. Stoddart A et al. 2012

Pharmacological DOT1L inhibition with EPZ004777 suppresses Hoxa and Meis1 and selectively impairs proliferation of CALM-AF10-transformed cells, and pretreatment reduces their in vivo spleen-colony-forming ability.

Murine, tool compound. The efficacy result does not measure what the same treatment does to the already-depleted global H3K79 pool or to genome stability, which is the concern this entry raises.

- DOT1L inhibition has never been tested clinically in this fusion: the pinometostat paediatric and adult trials enrolled on 'rearrangement involving the MLL gene, including reciprocal chromosomal translocations involving 11q23', and PICALM::MLLT10 breaks at 11q14. The clinical disappointment with DOT1L inhibitors is evidence about KMT2A-rearranged disease only. ClinicalTrials.gov 2026

THE SECOND, AND THE REASON THE READOUT MUST BE LOCUS-RESOLVED: the same fusion STRIPS H3K79 methylation genome-wide while adding it at HOXA, so a bulk measurement averages two opposite effects and can move either way for the wrong reason.
the cytoplasmic-sequestration account of global H3K79 loss fails in leukaemia cells, where the fusion is nuclear and still suppresses the mark regardless of clathrin
