Paraspeckles and NEAT1 — no KMT2A evidence at all, but a fusion oncoprotein in another leukemia uses NEAT1 as a differentiation brake

also called NEAT1 · paraspeckle · NONO SFPQ · differentiation brake lncRNA

Paraspeckles are nuclear droplets built around a single long RNA called NEAT1. In one leukemia the fusion protein switches NEAT1 off, and switching it back on is part of how the curative drug makes the cells grow up — but nobody has ever measured NEAT1 or looked at a paraspeckle in KMT2A-rearranged leukemia.

The same thing, for a scientist

Paraspeckles are ribonucleoprotein bodies nucleated by the architectural lncRNA NEAT1 with DBHS proteins NONO, SFPQ and PSPC1. Direct evidence in KMT2A-rearranged leukemia: **none**. A dedicated search (NEAT1 AND MLL leukemia; NEAT1 AND KMT2A; NEAT1[Title] AND lymphoblastic leukemia[Title]; NEAT1 paraspeckle leukemia) returned no study in a KMT2A-rearranged context, and produced one trap worth recording: PMID 34219124 is a **thymoma** RNA-seq paper that lists KMT2A-MAML2 among fusions and NEAT1 among differentially expressed lncRNAs in the same table and asserts no relationship between them; it must not be cited as a KMT2A-NEAT1 link. The indirect case rests on APL. Zeng et al. showed NEAT1 is low in de novo APL relative to healthy donors, is repressed in the presence of PML-RARA, rises during ATRA-induced differentiation of NB4 cells, and that siRNA knockdown of NEAT1 blocks ATRA-induced differentiation as measured by CD11b [E1]. That is a fusion oncoprotein using a nuclear-body lncRNA as a differentiation brake, in the disease this graph treats as the differentiation-therapy precedent. Two further AML studies report NEAT1 **decreased** in AML by ceRNA mechanisms [E2][E3], and a childhood ALL study places NEAT1 in a sponge network without KMT2A stratification [E4] — all bulk-expression correlations, none with paraspeckle imaging, none stratified by KMT2A status.

Everything needed to close the first-order question already exists in public data. NEAT1 is a standard-annotation lncRNA present in every RNA-seq dataset. The graph already carries the TARGET and St. Jude Cloud dataset entries. Asking whether NEAT1 is lower in KMT2A-rearranged infant ALL than in other paediatric ALL subtypes is a differential-expression query against existing counts, not an experiment. If the answer is yes, that is a novel observation connecting the two best-studied fusion leukemias through the same lncRNA; if no, the paraspeckle hypothesis is dead for a cost of nothing.

The second-order question needs microscopy: are paraspeckles present, enlarged, reduced or absent in t(4;11) blasts? NEAT1 RNA-FISH plus NONO immunofluorescence on cytospins of archival material would answer it. This is one of the few claims in this beat that fixed patient material can address, because paraspeckles are defined by an RNA that FISH detects directly rather than by a protein whose antibody cannot separate fusion from wild type.

The mechanistic bridge, and how weak it is

The APL result is real but its transfer to KMT2A rests on a shared phenotype (differentiation block) rather than a shared mechanism. PML-RARA is a nuclear receptor fusion that represses through RARE elements and corepressor recruitment; KMT2A::AFF1 is a chromatin-tethered elongation activator whose defining action is gene activation at CpG-island promoters. A repression mechanism transfers poorly between them. The graph's existing hyp-nuclear-speckle-paraspeckle-kmt2a-r states this analogy; this entry grades it at weak by analogy, and flags that the direction of the prediction is not even clear: if the fusion mainly activates, NEAT1 could as easily be up as down.

There is one route that would make the connection mechanistic rather than analogical. NEAT1 is a strongly Pol II-transcribed, short-half-life lncRNA whose abundance tracks elongation activity. A fusion that hijacks P-TEFb genome-wide changes the elongation budget. NEAT1 levels could therefore move as a by-product of SEC redistribution, with paraspeckle number following passively. That is a prediction with a sign — NEAT1 should track total SEC availability — and it is testable with the existing degron system.

The identifier trap, recorded deliberately

Searching 'NEAT1 AND KMT2A' in PubMed returns exactly one paper: a thymoma RNA-seq study (PMID 34219124) that happens to list KMT2A-MAML2 among detected fusions and NEAT1 among differentially expressed lncRNAs, in a different table, with no relationship claimed between them. It is not a leukemia paper and contains no KMT2A-NEAT1 finding.

This is exactly the failure mode that has produced identifier errors in this project before: a search returns a single hit, the hit contains both search terms, and the citation gets written as though the paper made the connection. The trap is recorded here so the next agent who runs this search recognises it.

How this could bite us back
  • If NEAT1 turns out to be required for menin-inhibitor-induced differentiation as it is for ATRA-induced differentiation, then blasts with low NEAT1 would be intrinsically refractory to differentiation therapy — a resistance mechanism invisible to every mutation-based resistance panel currently in use.
  • NEAT1 is stress-inducible and rises with proteotoxic and genotoxic stress, so any chemotherapy given alongside a differentiation agent perturbs it; a NEAT1-based biomarker would be confounded by the rest of the regimen.

  1. Is NEAT1 differentially expressed in KMT2A-rearranged infant ALL versus other paediatric ALL subtypes? A public-data query, answerable this week, and novel either way.
  2. Are paraspeckles present, enlarged or absent in t(4;11) blasts? NEAT1 RNA-FISH plus NONO immunofluorescence on archival cytospins is one of the few claims in this beat that fixed patient material can settle, because the compartment is defined by an RNA rather than by a protein whose antibody cannot distinguish fusion from wild type.
  3. Does NEAT1 abundance track SEC availability? If the fusion monopolises P-TEFb, NEAT1 — a Pol II-dependent, short-half-life lncRNA — should move when the fusion is degron-depleted, which converts a weak analogy into a mechanistic prediction with a sign.
  4. Does NEAT1 rise during menin-inhibitor-induced differentiation of KMT2A-r blasts, as it does during ATRA-induced differentiation in APL? If it does, NEAT1 is a candidate pharmacodynamic biomarker of differentiation that can be measured in blood.

NEAT1 expression is significantly lower in de novo acute promyelocytic leukemia patients than in healthy donors and is repressed in the presence of PML-RARA; NEAT1 rises during ATRA-induced differentiation of NB4 cells, and siRNA knockdown of NEAT1 impairs ATRA-induced myeloid differentiation as measured by CD11b surface expression.

    What this evidence cannot show

Patient qRT-PCR plus a single cell line (NB4) with transient siRNA; no paraspeckle imaging, no rescue, and the causal claim that PML-RARA represses NEAT1 rests on correlation with fusion status rather than on inducible fusion expression. APL, not KMT2A-rearranged leukemia.

Evidence that argues against this (1)
    • Decisive counter to any causal reading of paraspeckle involvement. Nakagawa et al. found NEAT1_1 and NEAT1_2 strongly expressed only in a particular subpopulation of cells in adult mouse tissues, with prominent paraspeckles only where NEAT1_2 is high, and then made a NEAT1 knockout mouse: the animals were viable and fertile under laboratory conditions with no apparent phenotype except that paraspeckles disappeared. Their own conclusion is that paraspeckles are non-essential, subpopulation-specific bodies formed secondary to particular environmental triggers. A body that can be abolished with no phenotype cannot be the mechanism by which a fusion oncoprotein works; at most it is a stress readout. Read: abstract only. Nakagawa S et al. 2011

NEAT1 is reported decreased in acute myeloid leukemia, acting through a NEAT1/miR-23a-3p/SMC1A axis in primary AML cells and THP-1.

    What this evidence cannot show

Correlative qRT-PCR with CCK-8, flow cytometry, RIP and dual-luciferase; competing-endogenous-RNA models of this design are weakly reproducible. THP-1 carries MLL-AF9 but the paper makes no MLL claim and does not stratify by fusion status.

Evidence that argues against this (1)
    • The measurement problem underneath E2, E3 and E4. NEAT1 produces two isoforms, and only the long NEAT1_2 is the architectural component of paraspeckles; the short NEAT1_1 completely overlaps its 5' end, so any qPCR or array probe placed in the shared region - which is what leukemia expression studies use - measures mostly NEAT1_1 and says nothing about paraspeckles. Adriaens et al. engineered cells and mice lacking NEAT1_1 but not NEAT1_2 and found no cell-cycle defect and none of the phenotypes seen in full NEAT1-deficient mice, concluding that NEAT1 function is attributable mainly or exclusively to NEAT1_2 and hence to paraspeckles. Therefore 'NEAT1 is down in AML' as reported by E2, E3 and E4 is not evidence that paraspeckles are down, and the paraspeckle strand of this entry is not supported by the expression data cited for it. What this evidence cannot show

Same ceRNA design class and same reproducibility concerns as E2; no KMT2A stratification and no paraspeckle measurement. Cited only to establish that NEAT1 has been looked at in AML generally and never in a KMT2A-rearranged context.

Evidence that argues against this (1)
    • NEAT1 expression changes should not be read as paraspeckle changes, and paraspeckles themselves are dispensable in a whole animal. Nakagawa et al. found that NEAT1_1 and NEAT1_2 are strongly expressed only in a particular subpopulation of cells in adult mouse tissues, with prominent paraspeckle formation only where NEAT1_2 is high, and that NEAT1 knockout mice are viable and fertile under laboratory conditions with no apparent phenotype other than the disappearance of paraspeckles - concluding that paraspeckles are non-essential, subpopulation-specific bodies formed secondary to particular environmental triggers. A qPCR-level down-regulation of NEAT1 in bone-marrow monocytes and two AML lines therefore says nothing about whether any paraspeckle existed in those cells, and the body whose absence is being inferred is one an organism can do without entirely. In 64 childhood acute lymphoblastic leukemia bone-marrow samples, NEAT1 and MALAT1 are proposed as sponges in a miR-335-3p/ABCA3 network.

    What this evidence cannot show

Correlative qRT-PCR in 64 patients with no KMT2A stratification, no infant subgroup and no imaging. This is the closest NEAT1 comes to paediatric ALL and it is not close.

Evidence that argues against this (1)
    • The two lncRNAs in this ceRNA network have both been deleted in mice, and the result was nothing. Nakagawa et al. generated Malat1-knockout mice by homologous recombination and found them viable and fertile with no apparent phenotype and correctly localised nuclear speckle markers, despite prior cell-culture work implicating Malat1 in migration, synapse formation, cell-cycle progression and serum response; the only detected change was reduced Neat1 levels in a subset of tissues and cells, which incidentally makes the two RNAs non-independent variables in any correlative panel that measures both. A 64-patient correlation in which NEAT1 and MALAT1 are proposed as sponges for a microRNA therefore rests on two transcripts whose loss is tolerated in vivo and whose levels are coupled to each other.
