Menin inhibition collapses the serine/one-carbon/ASNS programme in KMT2A::AFF1 cells - PHGDH -3.24, PSAT1 -2.40, ASNS -1.72, flat in a wild-type line - so it should re-sensitise KMT2A-rearranged ALL to asparaginase, a combination PubMed has zero records for
also called menin inhibitor plus asparaginase · ASNS collapse · serine biosynthesis KMT2A · PHGDH menin leukaemia
A menin-blocking drug switches off the machinery these leukaemia cells use to make their own amino acids, including the one gene that lets them survive the standard drug asparaginase. That predicts the two drugs should work better together, and nobody appears to have tried it.
The same thing, for a scientist
**The hypothesis:** in KMT2A-rearranged ALL, menin-MLL inhibition suppresses the ATF4-driven serine/one-carbon and asparagine-synthesis programme, and because asparaginase works by exploiting low ASNS, a menin inhibitor should re-sensitise these cells to asparaginase. **Five things it rests on, four of them already established and one measured here.** 1. **Menin drives serine biosynthesis, and menin-MLL inhibition suppresses it** - demonstrated in Ewing sarcoma, where MLL1/menin are overexpressed oncogenes and small-molecule disruption of the interaction causes loss of menin and MLL1 protein and inhibition of growth (PMID 29672864, already cited twice in this graph). The same group's follow-up places ATF4 at the centre of it (PMID 33741715). 2. **Measured here, in leukaemia, for the first time in this graph:** under VTP-50469 at 330 nM in RS4;11 at day 7, **PHGDH -3.24, PSAT1 -2.40, CBS -4.07, CTH -1.57, MTHFD2 -1.16, SHMT2 -1.09, GPT2 -1.54, SLC7A5 -1.34, SLC3A2 -1.27, SLC1A5 -0.99**, with ATF4 itself -0.86 and DDIT3/CHOP -1.50. In **REH**, the KMT2A-wild-type line, every one of these is flat (PHGDH +0.34, PSAT1 +0.49, ATF4 +0.07). 3. **ASNS falls with them: -1.72** (3.3-fold), against **+0.26 in the wild-type line**. 4. **ASNS level is what asparaginase exploits.** `gene-asns` in this graph already states it: asparaginase works against low ASNS, and resistance emerges through ASNS induction mediated in part by the amino-acid stress sensor **GCN2** - whose canonical effector is ATF4, which that entry never names. 5. **The target is worth hitting in this disease.** `gene-asns` records asparaginase resistance as the **second-largest in-vitro effect in the entire backbone at about 12-fold** in KMT2A-rearranged infant ALL, with the mechanism never measured in the disease. **And the combination is unpublished.** `menin[tiab] AND asparaginase[tiab]` returns **0**, parsed translation, `warninglist: ["No items found."]`, **run twice**. So does `(revumenib OR "menin inhibitor") AND asparaginase`. `ASNS AND KMT2A` returns 0; `ASNS AND MLL` returns exactly one record, and it points the same way: EVI1-driven metabolic reprogramming in MLL-rearranged AML **increases sensitivity to L-asparaginase** (PMID 31649131, uncited in this graph). **This is a hypothesis, not a result.** No asparaginase was given to any cell in either dataset. What is measured is transcript abundance of ASNS and the pathway feeding it, in two cell lines, under one compound, at one dose, at day 7. **It is early, which partly answers the obvious objection.** A whole biosynthetic programme falling at day 7 under an effective drug could simply be arrested cells. But at **day 2 only 19% of the day-7 response exists** (323 of 1,686 genes), and the serine arm is already down then - **CBS -1.86, PHGDH -1.08, PSAT1 -1.05, GPT2 -0.90, ASNS -0.84**, ATF4 -0.51 - moving with the canonical targets (MEF2C -3.29, MEIS1 -2.59, PBX3 -1.38). The downstream ISR *output* genes are late by contrast (TRIB3 13% of its d7 effect, ATF5 22%, SESN2 24%), giving a sensible order: ATF4 and the enzymes first, their stress output after. This bounds the arrest confound; it does not eliminate it, because 48 hours at 330 nM against a 20-60 nM IC50 is long enough for growth effects, and ASNS itself is only -0.84 at day 2.

| the panel was not pre declared | Unlike the HOXA/MEIS1 panel, which was written into a script before either dataset was read, this amino-acid-response panel was assembled AFTER the 24-gene concordance list pointed at ATF4. It is therefore a follow-up look and carries the corresponding discount. What partially offsets it: the panel is the textbook ATF4 target set rather than a list chosen from the data, the wild-type contrast was run on all of it, and the direction was predicted by PMID 29672864 before it was measured here. |

If forcing ASNS expression in an AFF1 line does not restore asparaginase resistance under menin inhibition, then ASNS is not the node through which the combination would work, even if the correlation holds." ]
- THIS IS A HYPOTHESIS FROM CELL-LINE TRANSCRIPTOMES AND MUST NOT REACH A PATIENT. No asparaginase was given to any cell in either dataset. The measurement is mRNA, in two cell lines, under one compound, at one dose, at a single late timepoint, with no replication in an infant-derived model and no protein or flux data.
- The population this would concern is infants, and asparaginase is already among the most toxic components of their backbone - pancreatitis, thrombosis, hypersensitivity and hepatotoxicity are dose-limiting and cumulative. A hypothesis that proposes making cells MORE sensitive to it is as much a de-escalation hypothesis as an intensification one, and the responsible version of the experiment asks whether menin inhibition allows LESS asparaginase, not more.
- Menin inhibitors carry differentiation syndrome and QT prolongation. Adding a drug class with its own severe toxicity profile to an already-toxic backbone is not a free combination, and nothing here addresses tolerability.
- The strongest counter-reading is in E1: an entire biosynthetic programme falling at day 7 under an effective drug is what arrested or dying cells look like. If that is what this is, the combination rationale evaporates, because the ASNS fall would be a marker of response rather than a druggable vulnerability.
- E3's own direction complicates the story: in MLL-rearranged AML, the oncogenic driver EVI1 INCREASES asparaginase sensitivity, so removing a driver programme could plausibly reduce it. The hypothesis survives only through ASNS specifically, and that step is assumed.
- Do not cite this entry as evidence that a menin inhibitor and asparaginase are synergistic. It is a prediction with a named experiment and an explicit list of ways it could be wrong.
- THE EXPERIMENT. Menin inhibitor (VTP-50469 or revumenib) plus asparaginase, in a dose matrix, in at least one infant-derived KMT2A::AFF1 line - this graph names seven, the youngest from a nine-day-old - with a KMT2A-wild-type line as the specificity arm, reading out viability, ASNS protein rather than transcript, and intracellular asparagine. The prediction is synergy in the fusion lines and not in the wild-type line.
- Is ASNS PROTEIN reduced? Everything here is transcript. ASNS protein and asparagine synthetase activity are what asparaginase sensitivity depends on, and a 3.3-fold mRNA fall need not translate.
- PARTLY ANSWERED the same cycle, see E5. At day 2 only 19% of the day-7 response exists, yet CBS (-1.86), PHGDH (-1.08), PSAT1 (-1.05) and ATF4 (-0.51) are already down, alongside MEF2C (-3.29) and MEIS1 (-2.59). So the serine arm is early rather than a late arrest signature. It does NOT eliminate the confound - 48 hours at 330 nM against a 20-60 nM IC50 is long enough for growth effects - and ASNS itself is only -0.84 at day 2. A growth-matched control or a 6-12 hour timepoint would settle it and neither exists in this series.
- Does menin inhibition suppress this programme in a KMT2A-wild-type line? The wild-type null here uses MI-2-2, the weak compound, which is the single biggest gap in this chain. No public dataset appears to contain a potent menin inhibitor applied to a KMT2A-wild-type leukaemia line.
- Does GCN2 status modify it? gene-asns names GCN2 as the sensor mediating ASNS induction and never names ATF4, its canonical effector. If menin inhibition lowers ATF4 output, the predicted interaction with GCN2-pathway status - and with the GCN2 inhibitors now in development - is direct and untested.
- Is the serine arm separately actionable? PHGDH -3.24 and PSAT1 -2.40 are larger than the ASNS effect, PHGDH inhibitors exist, and PHGDH is named 35 times in this graph with no entry of its own.

Under VTP-50469 at 330 nM in RS4;11 (KMT2A::AFF1) at day 7, the ATF4-driven amino-acid-response and serine/one-carbon programme falls coherently: CBS -4.07, PHGDH -3.24, PSAT1 -2.40, VEGFA -1.83, ASNS -1.72, CTH -1.57, GPT2 -1.54, DDIT3/CHOP -1.50, EIF4EBP1 -1.39, SLC7A5 -1.34, XBP1 -1.31, SLC3A2 -1.27, MTHFD2 -1.16, SHMT2 -1.09, SLC1A5 -0.99, with ATF4 itself at -0.86, all against the same-line same-day DMSO arm with n=3. In REH, the KMT2A-wild-type line in the companion dataset, the same genes are flat under MI-2-2 (PHGDH +0.34, PSAT1 +0.49, ASNS +0.26, ATF4 +0.07, MTHFD2 +0.41). Under the weak compound MI-2-2 in RS4;11 itself the programme barely moves (ASNS +0.17, PHGDH -0.44, PSAT1 -0.49).
What this evidence cannot show
This panel was NOT pre-declared: it was assembled after the 24-gene concordance list pointed at ATF4, so it is a confirmatory look and not a discovery screen, and it should be discounted accordingly relative to the pre-registered HOXA/MEIS1 panel. The wild-type contrast uses a DIFFERENT COMPOUND (MI-2-2) because GSE127506 has no KMT2A-wild-type line, and MI-2-2's whole-transcriptome effect in REH was small anyway - so 'flat in wild-type' is the weakest link in the chain. Transcript abundance is not flux: a fall in PHGDH and PSAT1 mRNA is not a measurement of serine synthesis, and a fall in ASNS mRNA is not a measurement of asparagine synthetase activity or protein. No p-values. Day 7 is late enough that a slowing or dying population could reduce biosynthetic transcripts as a consequence rather than a cause.
Evidence that argues against this (2)
- The whole programme falling together is as consistent with cells shutting down biosynthesis because they are arrested or dying as with a specific menin-dependent transcriptional effect. At day 7 under an effective drug, reduced translation and growth lower amino-acid demand, and the ATF4 axis reports exactly that. A growth-matched control, or the day-2 arm, would discriminate - and this evidence item uses day 7 because the effect sizes are larger, which is precisely the choice that makes the confound worse.
- ATF4 itself moves only -0.86, less than most of its targets. That is compatible with ATF4 acting post-transcriptionally (it is classically regulated by translation, not transcript level) but it also means this analysis has no direct evidence that ATF4 activity changed at all. The signature is being read off the targets.

The precedent that menin-MLL inhibition suppresses serine biosynthesis is established in a different fusion-driven cancer. In Ewing sarcoma, MLL1 (KMT2A) and menin are overexpressed and function as oncogenes, small-molecule inhibition of the menin-MLL interaction causes loss of menin and MLL1 protein expression and inhibits growth and tumorigenicity, and the mechanistic basis was investigated by Bru-seq and reported as regulation of the serine biosynthetic pathway by menin.
Menin regulates the serine biosynthetic pathway in Ewing sarcoma. · Svoboda LK, Teh SSK, Sud S, et al. · The Journal of Pathology 2019What this evidence cannot show
Abstract-level; the full text was not read, so which serine-pathway genes moved and by how much in that system is not established here. Ewing sarcoma is a different disease, lineage and fusion, and EWS-FLI1 has no menin-binding motif analogous to KMT2A's MBM - so the shared node in that system is menin itself rather than a menin-binding fusion, which is a different architecture from KMT2A::AFF1. Verified live by efetch 2026-09-22; already cited twice in this graph, both times in a sarcoma-risk context rather than as a mechanistic precedent.
Evidence that argues against this (1)
- If menin drives serine biosynthesis wherever menin is active, then menin inhibition should collapse this programme in cells with no KMT2A fusion too - and in the one wild-type line available it does not. Either the leukaemic version is fusion-dependent while the Ewing version is not, or the wild-type null is an artefact of testing it with the weak compound. Both readings are live and the experiment that separates them is named below.

In MLL-rearranged leukaemia, metabolic reprogramming by an oncogenic transcription factor has already been shown to change asparaginase sensitivity in the predicted direction. EVI1, overexpressed in adult and paediatric MLL-rearranged AML with very poor prognosis, triggers metabolic reprogramming associated with leukemogenesis and INCREASES sensitivity to L-asparaginase.
EVI1 triggers metabolic reprogramming associated with leukemogenesis and increases sensitivity to L-asparaginase. · Saito Y, et al. · Haematologica 2021What this evidence cannot show
Abstract-level, and it is the ONLY record returned by ASNS[tiab] AND MLL[tiab]. It is AML rather than infant B-ALL, EVI1 rather than menin, and the mechanism by which its metabolic reprogramming alters asparaginase sensitivity is not established from the abstract - it may not run through ASNS at all. It is cited here as evidence that the axis is live in MLL-rearranged disease, not as support for the specific mechanism proposed. Uncited elsewhere in this graph; verified live by efetch 2026-09-22.
Evidence that argues against this (1)
- The direction is the useful part and also the trap: EVI1 overexpression INCREASES asparaginase sensitivity, so in that system the oncogenic driver makes cells MORE vulnerable to asparaginase, not less. If the analogy held tightly it would predict that removing the driver's programme - which is what menin inhibition does here - makes cells LESS sensitive, the opposite of this hypothesis. The two are reconciled only through ASNS specifically, and that reconciliation is assumed rather than shown.

The combination is unpublished in any indexed title or abstract. menin[tiab] AND asparaginase[tiab] returns 0 with a parsed querytranslation and warninglist ['No items found.'], run twice. (revumenib[tiab] OR 'menin inhibitor'[tiab]) AND asparaginase[tiab] returns 0 with the same signature. ASNS[tiab] AND KMT2A[tiab] returns 0. For contrast, the components are each well covered: asparaginase[tiab] AND ATF4[tiab] returns 21, ASNS[tiab] AND ATF4[tiab] returns 59, and PHGDH[tiab] AND menin[tiab] returns 2.
What this evidence cannot show
A parsed zero bounds INDEXED TITLES AND ABSTRACTS ONLY. This project has already been caught by exactly this: a genuinely reported NUTM1 fusion partner returned a parsed zero because the fact lived in a review's table. A menin-inhibitor/asparaginase combination tested inside a drug-screen supplement, a conference abstract, a trial protocol or an unpublished negative result would not appear in these counts. So 'nobody has published it in a title or abstract' is the claim, not 'nobody has tried it'.
Evidence that argues against this (1)
- An absence of publications on a plausible and cheap combination, in a field with several approved and investigational menin inhibitors and a seventy-year-old chemotherapeutic, is weak evidence of novelty and might be evidence that it has been tried and did not work. Combination screens are routinely run and negative arms routinely go unreported.
- E5moderate confidence 3 of 5: One strong study, or several smaller consistent ones.database
The collapse is early, which argues against it being a late consequence of growth arrest. At day 2 in RS4;11 under VTP-50469, only 19% of the eventual day-7 transcriptome response is present (323 of 1,686 genes moving >=1 log2 at d7 already move at d2, out of 688 d2 movers) - yet the serine arm is already down: CBS -1.86, PHGDH -1.08, PSAT1 -1.05, GPT2 -0.90, ASNS -0.84, SLC7A5 -0.77, with ATF4 itself -0.51 and ATF3 -1.45. These move alongside the canonical menin targets at the same timepoint (MEF2C -3.29, MEIS1 -2.59, PBX3 -1.38, HOXA10 -0.88). Eight of 23 panel genes carry at least half of their day-7 effect at day 2: ATF3 (79%), MEF2C (75%), PBX3 (64%), STC2 (63%), HOXA10 (61%), ATF4 (60%), GPT2 (59%), SLC7A5 (57%). By contrast the downstream ISR OUTPUT genes are late - TRIB3 13%, ATF5 22%, SESN2 24%, DDIT3 24%, SHMT2 20%, MTHFD2 24% - giving a coherent order: ATF4 and the biosynthetic enzymes first, their stress-response output afterwards.
What this evidence cannot show
Day 2 is early for a 7-day experiment but it is not an immediate-early timepoint, so this bounds the arrest confound rather than eliminating it - cells can be slowing by 48 hours. The d2/d7 fractions are ratios of two noisy estimates and are unstable where the d7 effect is small; they are reported only for genes with a d7 effect above 0.5 log2. ASNS specifically is -0.84 at day 2, BELOW the 1 log2 threshold used everywhere else in this analysis, so the gene the hypothesis turns on is the weaker part of this evidence item. And an early effect is still transcript, not flux.
Evidence that argues against this (2)
- The temporal argument can be turned around. That the biosynthetic enzymes move with the canonical targets at day 2 is equally consistent with both being part of one direct transcriptional programme AND with both being early consequences of a proliferation arrest that begins before day 2 - VTP-50469's IC50 is 20-60 nM and the dose here is 330 nM, so growth effects at 48 hours are expected. What would settle it is a growth-matched control or a 6-12 hour timepoint, and neither exists in this series.
- If ATF4 loss at day 2 drove the serine enzymes, the enzymes should lag ATF4 rather than lead it - and PHGDH (-1.08), PSAT1 (-1.05) and CBS (-1.86) are all LARGER at day 2 than ATF4 itself (-0.51). That ordering fits a shared upstream cause better than a linear ATF4-to-enzyme cascade, and ATF4 is in any case regulated translationally rather than transcriptionally, so its mRNA is a poor proxy for its activity.

REPLICATED IN A THIRD LINE, BY ANOTHER LABORATORY - and one half of the original reading did not survive. In MV4;11 (KMT2A::AFF1) under VTP-50469 500 nM from GSE260456 (PMID 41634341, Neville/Gilan, a different group asking about PRC1.1 and transcriptional memory), with the panel copied verbatim from this entry: PHGDH -2.77, GPT2 -2.34, PSAT1 -2.17, CBS -1.89, SLC1A5 -1.61, ASNS -1.16, MTHFD2 -1.03, ATF4 -0.99, and the positive control collapsing (MEIS1 -3.52, MEF2C -5.64, PBX3 -3.04). Median housekeeping drift is -0.33, so adjusted: PHGDH -2.44, PSAT1 -1.84, ASNS -0.83. The DOT1L inhibitor is much weaker on the same genes. The serine collapse also survives PCGF1 and BCOR knockout. WHAT DID NOT REPLICATE: the ISR output genes - TRIB3 -0.52 against -3.10, ATF3 +0.22 against -1.84, SESN2 -0.52 against -1.42, DDIT3 +0.18 against -1.50 - so the honest label for the replicated programme is serine/one-carbon/amino-acid transport, NOT the integrated stress response.

ASNS - the gene this hypothesis turns on - is the WEAKEST replicated member at -1.16 raw and -0.83 after housekeeping adjustment. The serine enzymes upstream replicate far more strongly than the asparaginase-relevant step does. Housekeeping is not flat in that matrix (ACTB -0.73), the exposure duration is unstated, MV4;11 is myeloid/biphenotypic rather than B-lymphoid, and it is not infant-derived either. - The non-replication of TRIB3, ATF3, SESN2 and DDIT3 removes the framing this hypothesis was originally built on. The ATF4-signature identification came from a 24-gene concordance list in which those genes were central; two of them now reverse sign in an independent line. The hypothesis survives on the serine/ASNS arm, which is the part that matters for asparaginase, but the mechanistic story attached to it is weaker than when it was written this morning.
- A global housekeeping drift in the replicating dataset is itself evidence for the arrest explanation this hypothesis has never been able to exclude. Two independent datasets now show a coordinated fall in biosynthetic transcripts under an effective drug, and in one of them the housekeeping genes fall too.
ASNS falls 1.72 log2 under VTP-50469 in RS4;11 and +0.26 in the wild-type line; that entry already holds the other half -- asparaginase resistance is the second-largest backbone effect at ~12-fold, mechanism unmeasured
predicts re-sensitisation: menin inhibition lowers the ASNS that asparaginase exploits. PubMed has ZERO records for menin AND asparaginase, parsed and run twice
grew out of that entry ATF4 identification: naming the 24 concordant genes pointed at ATF4, and the serine/ASNS arm was then checked
- analogous to →EWSR1::FLI1 (EWS-FLI1), t(11;22)(q24;q12)
menin regulates the serine biosynthetic pathway in Ewing sarcoma (PMID 29672864) and menin-MLL inhibition suppresses it -- the precedent, in a fusion with no menin-binding motif
rests on the same VTP-50469 arm, and on the finding that the weak compound does not move this programme either (ASNS +0.17)
independent replication in MV4;11 from another lab: the serine arm holds (PHGDH -2.77, ASNS -1.16) and the ISR output genes do not (TRIB3 -0.52, ATF3 +0.22)
set up to decide durability vs reversibility and REFUSES: the washout control never recovers (median 0.208 at 96h), ASNS is undetectable in MOLM13, and the mouse arm never engages the serine programme
the combination this hypothesis proposes is ALREADY recruiting in infants (NCT05848687, PEG asparaginase + ziftomenib), so the therapeutic idea is not ours to propose - and the prediction independently lost its menin-selectivity when ASNS came back 86% reproduced by a DOT1L inhibitor
the serine arm of that hypothesis re-measured on a pre-registered panel; PHGDH -3.24 confirmed
re-measures that hypothesis's serine panel on a pre-registered panel and adds the MOLM13 arm, where the serine collapse is absent
corrects how that hypothesis should be read: the gene it turns on is the compound-independent one
