FUT4 - the myeloid alpha-1,3-fucosyltransferase, a hub gene of the KMT2A-rearranged expression module, and the gene HGNC once officially called CD15
also called FUT4 · ELFT · FCT3A · FUC-TIV · Fuc-TIV · FucT-IV · fucosyltransferase 4 · alpha (1,3) fucosyltransferase myeloid-specific · P22083
FUT4 is an enzyme that sticks a fucose sugar onto other sugars, and that reaction is what creates the CD15 marker on white blood cells. It is one of the few genes that a computer can actually look up which connects to CD15, a marker this project mentions two dozen times but could never trace back to a gene.
The same thing, for a scientist
FUT4 (UniProt **P22083**, FUT4_HUMAN, **530 aa**, single reviewed human record, primary gene name FUT4, synonyms ELFT and FCT3A) is the myeloid alpha-1,3-fucosyltransferase - HGNC's own alias name for it is *alpha (1,3) fucosyltransferase, myeloid-specific* [E1][E2]. It transfers fucose to the GlcNAc of a type-2 LacNAc acceptor, generating **Lewis-x (CD15)**. **Two facts make this gene the hinge of this whole beat.** 1. **`CD15` is a PREVIOUS OFFICIAL HGNC SYMBOL of FUT4** [E2]. The antigen that no gene encodes has, historically, been the official name of an enzyme gene. So a symbol-keyed instrument is not simply blind to CD15 - it can be **confidently wrong** about it, which is the worse failure. 2. **FUT4 is a hub gene of the co-expression module most positively correlated with MLL rearrangement** in 204 paediatric ALL patients, alongside PROM1, LGALS1, CD44 and HOXA10, and its expression is significantly increased in MLL-rearranged samples [E3]. This is the only result found on this beat that puts a CD15-building enzyme inside the KMT2A-rearranged programme. **And the biology warns against reading it simply.** In four myeloid lines spanning maturation stages, the Fuc-TIV (FUT4) mRNA peak at the HL-60 promyelocyte stage coincided with **upregulation of Lewis-x and VIM-2 and simultaneous downregulation of sialyl-Lewis-x** [E4]. FUT4 level does not set CD15 level; it shifts a competition for shared acceptors. A CD15 measurement is a flux outcome, not a gene-dosage readout. In this graph, measured 2026-09-20, **FUT4 appears in 2 node files against CD15's 24** [E5].
What FUT4 can and cannot be used for
Can. FUT4 is a real protein-coding gene with a UniProt accession and an HGNC id, so it is reachable by every instrument this project owns: DepMap, cBioPortal, a CRISPR screen, an expression query, the named-but-unwritten queue. That is the entire value of this entry - it converts a 24-entry antigen from unreachable to queryable, at the cost of an explicit approximation.
Cannot.
- FUT4 is not CD15. One is an enzyme, the other its product; several GT10-family enzymes can make the epitope [see
gene-fut9], and the epitope sits on many carriers. - FUT4 expression is not CD15 surface level 4.
- A FUT4 dependency result would not be a CD15 result. FUT4 fucosylates many acceptors; knocking it out removes far more than CD15.
- The hub-gene finding is network topology, not function 3. Nobody has knocked FUT4 down in a KMT2A-rearranged cell and asked what happens.

}- **FUT4 is a proxy for CD15 and must be labelled as one.** Any downstream analysis that silently substitutes FUT4 for CD15 reproduces the exact conflation this beat exists to correct.
- **Knocking out FUT4 strips selectin ligands too.** Its protein name is *ELAM-1 ligand fucosyltransferase*; interfering with it perturbs leukocyte trafficking and marrow homing, which in a leukaemia context is a large and unpredictable off-target effect.
- **Glycan output is competitive, not additive.** Raising FUT4 lowers sialyl-Lewis-x while raising Lewis-x [E4], so a FUT4 intervention has two opposite effects on two antigens this graph cares about.
- Targeting the CD15/Lewis-x epitope is not targeting FUT4, and targeting FUT4 would not remove the epitope. FUT7 and FUT9 make overlapping products in human leukocytes (PMID 23192350), and Lewis-x is a carbohydrate built by several enzymes - so an antibody strategy and an enzyme-inhibition strategy here have different targets and neither substitutes for the other.
- FUT4 constructs selectin ligands that neutrophils use to leave the bloodstream (PMID 23192350). Inhibiting it in a leukopenic child would impair neutrophil recruitment to infection - a leukocyte-adhesion-deficiency-like effect in a patient whose main non-relapse risk is infection.
- Does FUT4 knockdown remove surface CD15 from KMT2A-rearranged blasts? This single experiment would make a 24-entry antigen dependency-screenable and nobody has published it.
- Is FUT4's module membership in MLL-rearranged ALL a fusion effect or a myeloid-priming effect? Comparing FUT4 expression between KMT2A-r and non-KMT2A myeloid-antigen-positive B-ALL would separate them.
- What is FUT4's DepMap dependency profile in KMT2A-rearranged lines, lineage-split and reported as raw effect? Unknown; not run by this beat.
- Which enzyme actually builds the CD15 epitope measured on KMT2A-rearranged blasts - FUT4, FUT7 or FUT9? The three are separable by knockdown in a leukaemia line and the answer determines whether the expression-module hub is also the biosynthetic one.
- Is FUT4's position as a hub of the KMT2A-rearranged expression module a consequence of myeloid-antigen co-expression rather than of anything the fusion does directly? The module was defined on expression, and the DepMap placement at rank 16,678 says the gene is doing no work in these cells.

The `gene_exact:FUT4` query returned exactly one reviewed human row and its primary gene name equals FUT4, per rule 62. Canonical isoform length; FUT4 is a Golgi type-II membrane protein whose catalytic domain is a fraction of the chain.
Evidence that argues against this (1)
- The protein name 'ELAM-1 ligand fucosyltransferase' points at selectin-ligand synthesis rather than at CD15, and it is a reminder that this enzyme's best-characterised job in the literature is trafficking chemistry, not leukaemia phenotyping. Reading FUT4 primarily as 'the CD15 gene' is this beat's framing and not the field's. UniProt Consortium 2026

Fetched live from the HGNC REST API on 2026-09-20. **`CD15` being a previous symbol is a nomenclature fact, not a biological equivalence** - HGNC retired it precisely because the CD number names an epitope and the symbol must name a gene. An implementer who reads the previous-symbol list as a mapping from CD numbers to genes will get a wrong answer for CD15 and for every other glycan CD number.
Evidence that argues against this (1)
- An HGNC free-text search on 'CD15' scores FUT4 highly, so the false mapping is easy to produce accidentally and hard to notice, since it returns a real gene with a real accession. Contrast CD65, for which HGNC returns nothing at all - the honest blank is safer than the plausible wrong answer. HUGO Gene Nomenclature Committee 2026

The MLL-rearranged fraction of the 204 patients, the partner composition and the platform are not given in the abstract. HOXA10's presence in the module is expected for KMT2A-r and is a sanity check that the module is real; FUT4's presence is the novel part for this beat and is the least corroborated.
Evidence that argues against this (1)
- Co-expression modules in leukaemia expression data are heavily driven by lineage and maturation stage. KMT2A-rearranged blasts are myeloid-primed, and FUT4 is the myeloid-specific fucosyltransferase, so FUT4's module membership may simply restate that the blasts are myeloid-primed rather than reveal a fusion-driven regulation of FUT4. Fuc-TIV (FUT4) mRNA increased progressively across KG1a, KG1 and HL-60 cells to peak at the HL-60 promyelocyte stage, and that peak correlated with increased fucosyltransferase activity towards non-sialylated acceptors, transitory downregulation of surface sialyl-Lewis-x, and upregulation of Lewis-x, VIM-2 and Lewis-y.

Evidence that argues against this (1)
- Because FUT4 and FUT7 compete for overlapping acceptors, raising FUT4 lowers sialyl-Lewis-x while raising Lewis-x. So any intervention on FUT4 intended to modulate CD15 would simultaneously and oppositely modulate selectin ligands - which govern marrow homing. That is a reason not to treat FUT4 as a clean handle on CD15. Kadirvelraj R et al. 2023

Abstract read in full; full text not read. Human HL-60 - a MYELOID leukaemia line, not KMT2A-rearranged - plus murine neutrophils; the biology is leukocyte adhesion in inflammation, not leukaemogenesis (brief rule 73). The paper's own headline is that human and mouse DIFFER, so the murine knockout literature cannot be read across. FUT4's contribution is explicitly the LESSER one.
Evidence that argues against this (1)
- This is the evidence that FUT4 should not be treated as the gene behind a myeloid glycan phenotype. FUT7 predominates at PSGL-1 and FUT9 carries much of the E-selectin ligand activity that FUT4/FUT7 loss fails to remove in human cells. Combined with brief rule 69 - CD15 is a PREVIOUS OFFICIAL SYMBOL of FUT4 but FUT4 helps BUILD the Lewis-x epitope rather than encoding it - any inference running from a CD15 flow result to FUT4 passes through two separate over-attributions.
