DNTT (TdT) - a real gene the graph calls by its enzyme nickname, and TdT-negativity is 100% KMT2A-rearranged in the largest paediatric series
also called DNTT · TdT · TDT · terminal deoxynucleotidyl transferase · terminal transferase · DNA nucleotidylexotransferase · terminal addition enzyme · P04053 · EC 2.7.7.31
TdT is an enzyme inside immature lymphoid cells that adds random letters to DNA while antibody genes are being assembled, and it is checked on almost every leukaemia diagnosis. In the largest study of children whose leukaemia lacked it, every single one of the 21 cases had a KMT2A rearrangement.
The same thing, for a scientist
DNTT (UniProt **P04053**, TDT_HUMAN, **509 aa**, single reviewed human record, primary gene name DNTT, synonym TDT, HGNC:2983 with alias symbols TDT and TdT and no previous symbols) encodes terminal deoxynucleotidyl transferase, EC 2.7.7.31 [E1][E2]. **The finding.** In the largest series to date, drawn from St. Jude Total XV and XVI, **TdT-negative B-ALL (n=21) was 100% KMT2A-rearranged against 1.9% of TdT-positive B-ALL (n=896), P < 0.001**; TdT-negative cases had younger median age (1.4 vs 6.8 years), higher WBC (52.8 vs 9.9 x10^9/L), no hyperdiploidy, and inferior 5-year EFS (76.2% vs 90.3%, P = 0.047). Within KMT2A-rearranged B-ALL (n=38), TdT-negativity associated with the **MLLT1** partner (P = 0.026) and was not independently predictive of survival, so its high-risk features are secondary to the underlying rearrangement. And critically for a diagnostic panel: **63% of KMT2A-rearranged B-ALL cases not identified by NG2 were TdT-negative** [E3]. **That last sentence makes TdT the complement of this graph's best-covered marker.** NG2/CSPG4 is named in 59 node files; the antigen that catches nearly two thirds of the cases NG2 misses had no entry at all. **And TdT is the second, sneakier half of this beat's blind spot.** It is a *bona fide* protein-coding gene, so the named-but-unwritten queue could in principle see it. It does not, because the graph writes the **enzyme nickname**: measured 2026-09-20, **TdT appears in 14 node files and DNTT in 4**, a ratio of 3.5. A queue enumerating HGNC symbols sees 4 of 18 discussions, counts the topic as minor, and never surfaces it [E4]. **Nomenclature, not biology, decided that this entry did not exist.** **Not a surface antigen.** TdT is nuclear. It sits in this beat because it is measured on the same diagnostic panel, in the same decision, as CD15, CD65s and NG2, and because it is the clearest example of the naming failure the beat was commissioned to document.

Ratio 3.5x. The dispatch brief estimated 13 and 3, i.e. 4.3x; this independent run gives 14 and 4, i.e. 3.5x. The exact ratio is not the point and the two runs do not reconcile - the point is that most of the graph's discussion of this gene is invisible to an instrument keyed on its symbol, and both measurements agree on that by a wide margin 4.
The same asymmetry is in the literature. On PubMed, 2026-09-20: (terminal deoxynucleotidyl transferase OR TdT) AND (KMT2A OR MLL) returns 39 records; DNTT AND (KMT2A OR MLL) returns 2. Controls: KMT2A OR MLL alone returns 9,229. A symbol-keyed literature sweep would find 2 of 39 papers - a 20-fold undercount, and the decisive paper 3 is not among the 2.
Why this class is worse than the CD15 class. CD15 is honestly unreachable: there is no gene, and any instrument that admits this returns an honest blank. DNTT is reachable, returns a real gene with a real accession, and returns the wrong size of literature - a confident, plausible, four-entry answer where the truth is eighteen. A blank prompts a human to look; a small number does not.
What TdT-negativity means diagnostically, and its limits
- As a KMT2A flag it is highly specific and rare. 21 TdT-negative cases out of 917 B-ALLs, all 21 KMT2A-rearranged 3. High specificity, very low sensitivity - it will never be a screening test.
- It is complementary to NG2, not redundant with it. 63% of the NG2-missed KMT2A-rearranged cases were TdT-negative 3, and NG2 was negative in 8 MLL-AF4 patients in the adult series 5. Two imperfect markers whose misses do not coincide.
- It is partner-associated. TdT-negativity tracked with MLLT1 rearrangement 3, which makes it a candidate partner-level phenotype rather than a generic KMT2A one.
- It is not independently prognostic. Within KMT2A-rearranged B-ALL, TdT status did not predict survival 3; the inferior EFS of the TdT-negative group is the KMT2A rearrangement talking.
- And it is measurable today. The authors' own framing: implementable using existing standard acute leukaemia flow panels 3.
The tension with myeloid maturation logic. In AML, terminal transferase pairs with CD34, CD117 and CD65s-low blasts 6. The KMT2A-rearranged B-ALL phenotype is TdT-negative, CD34-negative and CD65s-positive 5 - the opposite pairing on both axes. A KMT2A-rearranged blast wears a combination normal myeloid maturation does not produce, and nothing in this graph explains it.

}- **A 100% from 21 cases is not a 100%.** The exact binomial lower bound on 21/21 is about 84%, and a single discordant case in the next series would change the clinical message.
- **TdT-negativity is a detection aid, not a risk factor.** It is not independently prognostic within KMT2A-rearranged B-ALL [E3]; using it to stratify treatment would be reading the rearrangement's risk off the wrong variable.
- **If TdT returns under a differentiating agent, it stops being a marker.** Any menin-inhibitor-driven change in blast maturation could alter TdT status, which matters for flow MRD assessment during therapy - untested.
- **The nickname problem is general and unmeasured.** This entry documents one instance found by hand; the queue that missed DNTT is still enumerating approved symbols without alias expansion, so the same failure is presumably live for other genes.
- There is no intervention here and this entry should not be read as proposing one. TdT is not a dependency in any DepMap arm (all values within 0.05 of zero), its function is repertoire diversification rather than survival, and TdT-null mice are healthy adults with an immature repertoire (PMID 8356452). Its value is diagnostic.
- As a diagnostic marker the failure mode is the panel, not the biology. TdT is nuclear, so reading it requires permeabilisation on a flow panel otherwise measuring surface antigens, and 63% of the KMT2A-rearranged cases missed by NG2 are TdT-negative (PMID 34148065) - so two markers fail on overlapping cases and a panel using both is less independent than it looks.
- What is the joint sensitivity of NG2-positivity OR TdT-negativity for KMT2A rearrangement in one paediatric cohort? The 63% complementarity figure implies it is substantially better than either alone, and it has never been reported as a combined statistic.
- Why is TdT-negativity associated specifically with the MLLT1 partner? This graph has a `gene-mllt1` and a `fusion-kmt2a-mllt1` entry and neither records a TdT phenotype; a partner-specific loss of a V(D)J enzyme is a mechanistic claim worth testing.
- Is *DNTT* silenced transcriptionally by the fusion, or is TdT-negativity a consequence of the blast arresting before TdT is normally expressed? Those predict different things about whether TdT returns under menin inhibition.
- How many other genes in this graph are undercounted by the same nickname mechanism? TdT/DNTT was found by hand; nobody has swept for the general case.
- When does DNTT switch on in HUMAN B-lymphoid development, relative to birth? The perinatal timing is murine, and if the human switch is also perinatal then TdT-negativity in infant KMT2A-rearranged B-ALL may be a developmental-stage readout rather than a fusion effect - which is testable against normal human fetal and neonatal progenitors.
- Why is TdT-negativity associated specifically with the MLLT1 partner (P = 0.026)? If TdT-negativity were purely a stage-of-origin effect it should be partner-independent, so the partner association is the observation that discriminates the developmental hypothesis from a fusion-specific one.

Evidence that argues against this (1)
- The UniProt protein name contains four different names for the enzyme and none of them is 'TdT', which is the string the clinical literature and this graph actually use. Fetched live from the HGNC REST API on 2026-09-20. **HGNC does carry TdT as an alias symbol, so the mapping TdT -> DNTT is resolvable and correct** - unlike CD15, whose mapping to FUT4 is resolvable and wrong, and CD65, which is unresolvable. That makes the DNTT undercount purely an instrument-design failure: the named-but-unwritten queue enumerates approved symbols and does not expand aliases, so a correct, freely available mapping goes unused.
Evidence that argues against this (1)
- Because the alias is available, the fix here is trivial (expand aliases before counting) while the fix for CD15 is impossible. Treating the two as one problem understates how easy this half is to repair and how hard the other half is. HUGO Gene Nomenclature Committee 2026

Abstract read in full from raw NCBI efetch XML on 2026-09-20; full text not read. **n=21 TdT-negative cases is a very small numerator for a '100%' claim** - a single TdT-negative non-KMT2A case would move it to 95%, and the 95% confidence interval on 21/21 extends down to roughly 84%. Single institution, two consecutive protocols, so referral pattern and TdT assay threshold are not independently replicated. The 5-year EFS difference is P = 0.047, borderline. The MLLT1 association rests on 38 KMT2A-rearranged cases split by TdT status.
Evidence that argues against this (1)
- The paper's own most important negative is that TdT-negativity was NOT independently predictive of survival within KMT2A-rearranged B-ALL, which means its association with poor outcome is entirely explained by the rearrangement. In-graph counts by Python regex with word boundaries (`\bTdT\b`, `\bDNTT\b`) over all node JSON on 2026-09-20; the TdT pattern is case-sensitive as written and would miss 'TDT', so 14 is a lower bound. **The dispatch brief quoted 13 and 3 (4.3x); this run gives 14 and 4 (3.5x) and the difference is unreconciled** - both establish a large undercount and neither exact figure should be quoted alone. PubMed counts from E-utilities esearch with `querytranslation` read back; note that PubMed expands 'terminal deoxynucleotidyl transferase' through a MeSH/Supplementary Concept route while `DNTT` expands through 'dntt protein human', so the two are not symmetrically indexed and part of the 39-vs-2 gap is indexing rather than authorship. **Rule 67 control:** `KMT2A OR MLL` alone = 9,229; no zero is recorded in this entry.
Evidence that argues against this (1)
- PubMed's expansion of the spelled-out enzyme name into MeSH terms inflates the 39 relative to a like-for-like string comparison, so the literature gap is real but smaller than 20-fold. The in-graph 3.5x ratio, where no expansion occurs on either side, is the cleaner measurement of the same phenomenon. Q48-surface-antigens 2026

- Combining a paediatric TdT result with an adult NG2 result to argue complementarity is exactly the kind of cross-population assembly this project warns against. The joint sensitivity of TdT-negativity plus NG2 has never been measured in one cohort, and the 63% figure in [E3] is the only direct evidence of complementarity. Klairmont MM et al. 2021

Low expression of the myeloid differentiation antigen CD65s, a feature of poorly differentiated AML in older adults: study of 711 patients enrolled in ECOG trials. · Paietta E, Neuberg D, Bennett JM, Dewald G, Rowe JM, Cassileth PA, et al. · Leukemia 2003What this evidence cannot show
Abstract read in full from raw NCBI efetch XML on 2026-09-20; full text not read. Adult AML, trials enrolling 1986-1999, not KMT2A-stratified; the abstract states the CD65s-low and CD65s-high groups did not differ in cytogenetic prognostic group distribution. Cited only to establish the normal myeloid maturation pairing of TdT with CD34 and CD65s-low, which the KMT2A-rearranged B-ALL phenotype inverts.

- This is AML and the contrast drawn in this entry is with B-lineage ALL, so the 'inverted pairing' observation compares two different lineages and may reflect lineage rather than any KMT2A-specific anomaly. It is recorded as an unexplained observation, not as a finding. Klairmont MM et al. 2021

In MICE, terminal deoxynucleotidyl transferase is the enzyme that adds template-independent N nucleotides during V(D)J rearrangement of lymphocyte receptor genes: receptor genes from adult TdT-mutant mice have few N nucleotides, proving the enzyme is essential for that junctional diversity, and they instead show extensive evidence of homology-directed recombination, suggesting TdT blocks that process. The developmental timing is explicit - SWITCH-ON OF THE TdT GENE DURING THE FIRST WEEK AFTER BIRTH provokes an even greater expansion of lymphocyte receptor diversity than had previously been thought.
Mice lacking TdT: mature animals with an immature lymphocyte repertoire. · Gilfillan S, Dierich A, Lemeur M, Benoist C, Mathis D · Science 1993What this evidence cannot show
Abstract read in full; full text not read. MURINE germline mutant, normal lymphocyte development - not leukaemia, not human, not KMT2A-rearranged (brief rule 73). The claim is about DEVELOPING LYMPHOCYTES generally (the abstract says 'lymphocyte receptor genes', covering both B and T) and does not resolve a single progenitor stage. The perinatal switch-on is a mouse developmental timing; the human equivalent is not established by this paper.
Evidence that argues against this (1)
- In THIS DISEASE the informative observation is the ABSENCE of the marker, which inverts the usual reading. In the largest paediatric series, TdT-NEGATIVE B-ALL (n=21) was 100% KMT2A-rearranged against 1.9% of TdT-positive B-ALL (n=896), with younger age (median 1.4 versus 6.8 years), higher WBC and inferior 5-year EFS (76.2% versus 90.3%); within KMT2A-rearranged B-ALL (n=38) TdT-negativity was associated with the MLLT1 partner and was NOT independently predictive of survival, the authors concluding the high-risk features are secondary to the underlying KMT2A rearrangement. So a gene whose normal job is a perinatal lymphoid-development function is, in this disease, most informative when it is switched off. Klairmont MM et al. 2021

TdT is expressed in immature lymphoid precursors and acts at V(D)J junctions during receptor rearrangement; the pro-B stage is where B-lineage V(D)J recombination occurs (murine normal lymphocyte development; PMID 8356452)
- expressed in →Pre-B cell — the stage KMT2A::AFF1 leukaemia never reaches, and why that boundary is informative
TdT-dependent N-nucleotide addition marks the B-lineage precursor stages; TdT-null mice have few N nucleotides in adult receptor genes (murine; PMID 8356452)

the murine knockout result covers lymphocyte receptor genes in both lineages, so the T-precursor expression is part of the same claim rather than a separate one (murine; PMID 8356452)
- related to →The neonatal window — the state in which an MLL fusion can transform, bounded at both ends
the TdT gene SWITCHES ON in the first week after birth in mice, so a fetal or neonatal lymphoid progenitor is TdT-low by developmental schedule - which would produce TdT-negative infant B-ALL (median age 1.4 years, 100% KMT2A-rearranged) without the fusion repressing anything (PMIDs 8356452, 34148065)
