ATR - the replication-stress kinase with the best preclinical case in this disease and the best published reason not to inhibit it
also called ATR · FRP1 · ataxia telangiectasia and Rad3-related protein · serine/threonine-protein kinase ATR
ATR is the alarm a cell sounds when its DNA copying machinery stalls. Blocking it kills KMT2A-rearranged leukaemia cells in mice, and the same alarm is also what stops early KMT2A-rearranged cells from becoming leukaemia in the first place - so the drug and the danger are the same switch.
The same thing, for a scientist
ATR (Q13535, 2644 aa) is the PIKK that senses replication stress, distinct from its DSB-sensing sibling ATM. Three lines place it squarely in this disease and they do not all point the same way. **For inhibition:** ATR inhibition induced chromosomal breakage and death in mouse AML(MLL) cells carrying MLL-ENL plus activated N-RAS independently of p53, and showed single-agent antitumoral activity in an immunocompetent allograft and in a human AML-MLL xenograft [E2]; in the lymphoid index disease, dual Mek/Atr targeting was potently synergistic in Mll-Af4/N-Ras^G12D mouse and patient-derived xenografts [E3]. **Against inhibition:** the ATR/ATM-CHK1/CHK2-p53/p21 checkpoint is a fail-safe senescence barrier at early stages of MLL-ENL transformation, and experimentally inhibiting that barrier **accelerated** the transition to immature cell states and acute leukaemia [E4]. **And orthogonally:** ATR already has a mechanistic role at KMT2A itself - it phosphorylates KMT2A at S516 to execute the S-phase checkpoint, a switch this graph holds at `mechanism-atr-kmt2a-s516-skp2-switch`. This entry also records a live instance of Rule 62: `gene_exact:ATR` returns **three** reviewed human records and the real ATR is **third** [E1].
Rule 62, caught live on this symbol
gene_exact:ATR AND organism_id:9606 AND reviewed:true returns three reviewed human records:

A script that takes the first data row gets alpha-1-antitrypsin-related protein, a 421-residue serpin. A script that takes the first row whose protein name looks plausible gets nothing, because neither impostor's protein name resembles a kinase - which is the easy version of this trap. The hard version is HDAC7/HDAC9, where the protein names collide too. The defence is the same either way: request gene_primary and length, keep only the row whose primary name equals the symbol, and assert both in the entry 1.
The therapeutic case, and the paper that inverts it
Morgado-Palacin et al. make the strongest single-agent case in this literature: AML(MLL) cells are resistant to genotoxic therapy because of an attenuated p53 response, and ATR inhibition kills them independently of p53, reducing established tumour burden and preventing tumour growth in an immunocompetent allograft and a human AML-MLL xenograft. They report that ATM inhibition also promoted survival of the AML(MLL) mice 2. Chu et al. extend it to the lymphoid index disease: in a new Mll-Af4/N-Ras^G12D mouse B-ALL, combined Mek and Atr inhibition was potent and synergistic in both mouse and patient-derived xenografts 3.
Takacova et al. describe the same pathway doing the opposite job at a different stage. In a regulatable MLL-ENL-ER mouse, oncogene activation induces an ATR/ATM-CHK1/CHK2-p53/p21 checkpoint and cellular senescence as a fail-safe anticancer barrier, and experimental inhibition of the barrier accelerated the transition to immature cell states and acute leukaemia development 4.
These are not contradictory - they are stage-dependent. The barrier operates during transformation; the vulnerability operates in established disease. But the conjunction has a specific consequence for this patient population and it belongs in bite_back: an ATR inhibitor given to a child with KMT2A-rearranged leukaemia is being given to a person who retains normal haematopoietic progenitors in which the same barrier is the thing preventing a second transformation.
ATR's other job in this graph: it phosphorylates KMT2A itself
mechanism-atr-kmt2a-s516-skp2-switch already holds the S-phase checkpoint switch in which ATR phosphorylates KMT2A. That is a separate relationship from the therapeutic one and this entry does not restate it; read it there. Its relevance here is that ATR inhibition in a KMT2A-rearranged cell perturbs both the checkpoint and a post-translational control on the wild-type KMT2A allele, and no published experiment separates the two.

}- The same kinase whose inhibition kills established KMT2A-rearranged leukaemia is part of the checkpoint that stops an early KMT2A-rearranged clone from becoming leukaemia. Giving an ATR inhibitor to an infant removes that barrier in every residual normal and pre-leukaemic progenitor at the same time (E4).
- ATR inhibitors are replication-stress agents and the patients are infants with decades of remaining lifetime in a disease whose origin is a DNA double-strand break. The second-malignancy argument must be made explicitly rather than assumed away, and no paediatric ATR-inhibitor safety dataset of the required duration exists.
- Both KMT2A-rearranged ATR results depend on a co-occurring activating RAS mutation. In infant t(4;11) RAS mutations are subclonal and frequently lost at relapse, so the population the combination is indexed on may be precisely the population that does not relapse.
- ATR is essential for replication in all proliferating tissue; complete loss is embryonic lethal and hypomorphic loss in humans causes Seckel syndrome. The therapeutic window here is a matter of degree of inhibition, not of target selectivity.
- Does ATR inhibition in a KMT2A-rearranged system act through the checkpoint, through the KMT2A S516 phosphosite, or both? Nobody has run an ATR inhibitor against a KMT2A S516A knock-in, which would separate them in one experiment.
- Is the senescence barrier (E4) intact in human fetal liver progenitors carrying a KMT2A fusion? If it is, an ATR inhibitor given to an infant is removing the brake on any residual pre-leukaemic clone at the same time as it kills the established one.
- Both published KMT2A-r ATR results carry an activating RAS lesion in the model. Is ATR inhibition effective in a RAS-wild-type KMT2A::AFF1 system at all? The two papers between them do not answer this, and infant t(4;11) is RAS-mutant only subclonally.

A naming fact. It is recorded at this length because it is a live instance of the failure mode this project's Rule 62 exists for, on a symbol used in almost every entry in this beat.
Evidence that argues against this (1)
- Reading the protein name back would have caught this particular swap, because neither impostor is a kinase. That defence is not general: the HDAC7/HDAC9 case documented in this project's brief has both records' protein names containing 'HD7', so name-reading returns something that looks right. Only gene_primary separates them, and only length catches a swap afterwards. UniProt Consortium 2026

Inhibition of ATR induced chromosomal breakage and death of mouse AML(MLL) cells carrying an MLL-ENL fusion and constitutively active N-RAS independently of p53, and ATR inhibition as a single agent reduced tumour burden after establishment and prevented tumour growth in an immunocompetent allograft model of AML(MLL) and in xenografts of a human AML-MLL cell line; ATM inhibition also promoted survival of the AML(MLL) mice.

Mouse AML with MLL-ENL plus activated N-RAS, and one human AML-MLL line xenograft. Myeloid, not the infant lymphoid index disease, and the N-RAS co-lesion is part of the model rather than a universal feature. The rationale rests on attenuated p53 signalling in AML-MLL, which is a property of that model system as much as of the disease.
Evidence that argues against this (1)
- The same ATR/ATM-CHK1/CHK2-p53/p21 axis acts as a fail-safe senescence barrier at EARLY stages of MLL-ENL transformation, and experimental inhibition of that barrier accelerated the transition to immature cell states and acute leukaemia development in vivo. The synergy is demonstrated in a genotype defined by Mll-Af4 PLUS activating RAS. RAS mutations are frequent in KMT2A::AFF1 B-ALL but subclonal and frequently lost at relapse in the sequenced infant cohorts, so a combination selected on RAS status may not survive the clonal dynamics of the disease it is designed for.
Evidence that argues against this (1)
- Infant t(4;11) B-ALL has the most silent mutational landscape reported for any sequenced paediatric cancer, with recurrent mutations found exclusively in KRAS and NRAS, subclonal and frequently lost at relapse. Activation of a regulatable MLL-ENL-ER oncogene in mice induces a fail-safe mechanism mediated by the DNA damage response that activates the ATR/ATM-Chk1/Chk2-p53/p21 checkpoint and cellular senescence at early stages of transformation; experimental inhibition of this barrier accelerated the transition to immature cell states and acute leukaemia development, indicating the DDR is a rate-limiting event for acquisition of stem-cell-like properties.

Mouse, MLL-ENL, regulatable oestrogen-receptor-fusion system, and the barrier is observed during transformation rather than in established leukaemia. It is not a statement that ATR inhibition causes leukaemia in a patient; it is a statement that in this model the checkpoint is tumour-suppressive at the stage a second malignancy would arise.
Evidence that argues against this (1)
- The therapeutic results run the other way in established disease, where ATR inhibition kills AML-MLL cells independently of p53 and shows single-agent activity in vivo. Three cell lines, western blot and qRT-PCR, no in vivo arm, no primary patient material, single group, published in Chinese. Evidence that argues against this (1)
- The companion chidamide + menin-inhibitor result places the disruption specifically at CHK1 and CHK2 and states that their upstream kinases ATR and ATM were NOT disrupted. Two combinations in the same disease, from different groups, therefore disagree about whether the upstream PIKKs move - which means neither should be cited as establishing an ATR-level effect. ATR's PML-nuclear-body and intranuclear-redistribution annotations rest on a dedicated localisation study: Barr et al. 2003 show that ATR KINASE ACTIVITY regulates the intranuclear translocation of ATR and of RPA following ionising radiation, and UniProt cites it at ECO:0000269 for the statement that, depending on the cell type, ATR can also be found in PML nuclear bodies.
ATR kinase activity regulates the intranuclear translocation of ATR and RPA following ionizing radiation. · Barr SM, Leung CG, Chang EE, Cimprich KA · Curr Biol 2003What this evidence cannot show
Rule 91 check performed and PASSED: the paper's title is a localisation claim and it is the source of the annotation. Human cultured cells with ionising radiation; the PML-body observation is explicitly CELL-TYPE-DEPENDENT in UniProt's own wording, so it is not a universal address. No haematopoietic material. The kinase-dependence of the redistribution is the useful part for this graph: it means an ATR inhibitor is predicted to change ATR's own subnuclear distribution, which is a pharmacodynamic readout nobody uses.
Evidence that argues against this (1)
- Cell-type-dependent PML-body localisation is a weaker claim than a compartment assignment, and UniProt's phrasing preserves the hedge while a downstream reader usually will not. No cell type in which ATR is PML-body-localised is haematopoietic, and no one has looked in a blast. The safer statement is that ATR's intranuclear distribution is dynamic and kinase-regulated, which the same paper supports directly. UniProt Consortium 2026

ATR is recruited to CHROMATIN during S phase - the annotation and the paper agree exactly, the paper's title being 'Recruitment of the cell cycle checkpoint kinase ATR to chromatin during S-phase'.
Recruitment of the cell cycle checkpoint kinase ATR to chromatin during S-phase. · Dart DA, Adams KE, Akerman I, Lakin ND · J Biol Chem 2004What this evidence cannot show
Rule 91 PASSED. Chromatin fractionation in human cultured cells; not haematopoietic and not fusion-bearing. This is the annotation that matters most for reading ATR inhibitor data in a rapidly cycling blast, because it says the relevant ATR pool is cell-cycle-phased, and a bulk whole-cell measurement of ATR or of its substrates averages over that.
Evidence that argues against this (1)
- The 'Chromosome' annotation in the same record rests on a paper that is NOT about ATR's localisation at all - Ward and Chen 2001 report that histone H2AX is phosphorylated in an ATR-dependent manner under replicational stress, which is a SUBSTRATE result. A reader counting experimental support for ATR-on-chromatin would count that paper and should not; the S-phase recruitment paper is the one that carries the claim. Ward IM et al. 2001

ATR mediates a checkpoint AT THE NUCLEAR ENVELOPE in response to mechanical stress, and separately relocalises to the MICRONUCLEAR envelope in response to genome instability - so ATR has an envelope-resident pool that is distinct from its chromatin and focal pools.

Human and murine cultured cells under mechanical confinement and migration; not haematopoietic. The micronuclear-envelope element comes from a separate reference in the same UniProt note and is not asserted from this paper. Included because the nuclear envelope is a compartment this graph has an open question about in KMT2A-rearranged cells and because leukaemic blasts migrate through confined marrow and vascular spaces, which is the stimulus class this checkpoint responds to.
Evidence that argues against this (1)
- A mechanically induced checkpoint characterised in migrating adherent cells may not operate in a suspension haematopoietic cell at all, and nobody has tested it. The envelope pool is also the newest part of ATR's record - the three supporting references are from 2014 and 2023 - so it has had far less independent replication than the chromatin and focal pools. The envelope relationship is bidirectional and enzymatic: DNA damage induces NUCLEAR ENVELOPE RUPTURE through ATR-mediated phosphorylation of LAMIN A/C.

Human cells; no leukaemia and no fusion. Recorded because it converts ATR's envelope annotation from 'is found there' into 'acts on the lamina', which is the difference between a location and a mechanism. This graph holds localization-nuclear-lamina-lads-kmt2a-r as an open question about lamina-associated domains in KMT2A-rearranged cells, and an ATR inhibitor is therefore predicted to change lamina phosphorylation in any cell it is given to - a confound for anyone reading LAD changes after ATR inhibition as a chromatin effect.
Evidence that argues against this (1)
- Lamin A/C is poorly expressed in most haematopoietic cells, which are lamin-B-dominant, so an ATR-lamin A/C mechanism established in adherent cells may have little purchase in a blast. The prediction in this item should be treated as a hypothesis to test rather than a confound to assume, and the test is a lamin isotype blot in the relevant cells. Kumar A et al. 2014

Not every experimentally coded reference in ATR's UniProt block is a localisation study. The 'Chromosome' annotation cites Ward and Chen 2001, a paper about ATR-dependent H2AX phosphorylation under replicational stress; and the note's statement that ATR 'redistributes to discrete nuclear foci upon DNA damage, hypoxia or replication fork stalling' cites Bass et al. 2016, whose subject is ETAA1 acting at stalled replication forks.

Rule 91, in the mild form and recorded for calibration rather than repair. Both cited papers are real, on the right pathway, and almost certainly contain an ATR imaging or fractionation panel somewhere - which is exactly what an ECO:0000269 code means and all it means. Neither is the founding observation for the annotated fact. Unlike MED26, HOXA10 or STAG2 in this tranche, nothing is LOST here, because the founding papers are also cited: the record is over-supported rather than mis-supported.
Evidence that argues against this (1)
- The generous reading is that a curator citing several papers for one compartment is recording breadth of support, not asserting that each paper is a localisation study, and that reading is probably right for ATR. It is recorded because the SAME audit applied to five other proteins in this tranche found records with no experimental code at all, and the contrast is only meaningful if the well-supported records are read with the same scepticism. Mouse spermatocyte chromosome spreads - a system with no counterpart in a leukaemic blast. Recorded because it is the founding observation that ATR is chromosome-associated at all, because it establishes the ATR/ATM distinction spatially rather than biochemically, and because a reader assuming the nine ECO:0000269 nuclear references are nine somatic cell-biology papers would be wrong about at least this one.
Evidence that argues against this (1)
- Meiotic chromosome axes are not mitotic chromatin, and the sites Atr and Atm occupy in a spermatocyte say nothing about their distribution in an S-phase blast. This item bounds the record's provenance; it should not be used as support for any somatic claim. Dart DA et al. 2004

THE ATR-CHK1 PATHWAY IS THE ONLY PUBLISHED UPSTREAM REGULATOR OF MENIN'S CHROMATIN-BOUND POOL. In HEK293 cells fractionated into soluble, two chromatin and nuclear-matrix components, menin in the chromatin fraction peaked at 4 h after UV irradiation and rose dose-dependently to a 2.8-FOLD maximum; caffeine pretreatment, an ATR-pathway inhibitor, blocked it, and the pathway was probed further with full-length and truncated-active CHK1 constructs.
Menin localizes to chromatin through an ATR-CHK1 mediated pathway after UV-induced DNA damage. · Farley SM, Chen G, Guo S, Wang M, A J, Lee F, Lee F, Sawicki M · J Surg Res 2006What this evidence cannot show
HEK293 cells, UV as the stimulus, and CAFFEINE as the ATR inhibitor - caffeine is not selective for ATR and also inhibits ATM and other PIKKs, so the attribution to ATR rests on the CHK1 constructs and on pathway logic rather than on a selective tool compound. Published in a surgical-research journal in 2006 and, per this project's citation-reuse check, cited in exactly ONE entry of this graph before now. It is not a leukaemia paper and there is no fusion anywhere in it. Confidence MODERATE for those reasons, and it is recorded here because it is the mechanistic hinge between this entry and the project's central protein.
Evidence that argues against this (1)
- A 2.8-fold shift in a fractionation from one 2006 study in HEK293 cells, with a non-selective inhibitor, is thin ground for a claim about menin pharmacodynamics in infant leukaemia, and this entry does not make one. The honest status is that the experiment the cycle-42 king log asked for - does DNA-damaging chemotherapy move menin onto chromatin in a KMT2A-rearranged blast - has a published protocol and a published precedent and has never been run in the right cells. The alternative explanation that would have to be excluded is that UV changes chromatin accessibility globally and menin follows passively. UniProt Consortium 2026

Deleting ATR in ADULT MICE causes acute cellular loss in tissues that require continuous proliferation, with dramatic reductions in TISSUE-SPECIFIC STEM AND PROGENITOR CELLS and exhaustion of tissue renewal and homeostatic capacity; the phenotypes include THYMIC INVOLUTION, hair greying, alopecia, kyphosis, osteoporosis and fibrosis - i.e. the premature appearance of age-related phenotypes.

Murine, conditional deletion in normal adult tissues, no leukaemia and no fusion. Complete genetic ablation is a harsher perturbation than pharmacological inhibition, which is partial and reversible - the scope objection this graph applies to gnomAD constraint applies here too. The haematopoietic readout emphasised is thymic involution rather than a direct HSC assay, so the bone-marrow stem-cell claim is by extension from the paper's general tissue-stem-cell finding.
Evidence that argues against this (1)
- The therapeutic literature in this very entry reports the opposite practical outcome: ATR inhibition as a SINGLE AGENT reduced established tumour burden in an immunocompetent AML(MLL) allograft and in a human AML-MLL xenograft, i.e. the animals tolerated it well enough for an antitumour effect to be the reported result. Germline-conditional deletion and a dosed inhibitor are different perturbations, and this graph should not use the knockout phenotype to predict clinical toxicity without saying which it is talking about. What survives is narrower and still worth carrying: the compartment at risk is the proliferating stem and progenitor pool, and the patients are infants. Farley SM et al. 2006
