Is fusion intended in biology sometimes? Yes, and the answer has a strategic edge

also called programmed recombination · physiological gene fusion · intended fusion

Deliberately breaking and rejoining DNA is a normal part of being a vertebrate: it is how antibodies are built, how antibody classes are switched, and how eggs and sperm are made. Healthy cells also routinely make hybrid RNA messages without touching the DNA at all.

The same thing, for a scientist

This entry answers one of the PI's questions directly and then draws the conclusion he was reaching for. Programmed genome breakage and rejoining occurs in at least three physiological settings — V(D)J recombination, class switch recombination and meiotic recombination — and chimeric transcripts occur in healthy tissue by at least two mechanisms that never touch the genome, trans-splicing and read-through (cis-SAGe) transcription. Each has its own entry with primary evidence. The strategic point is that **machinery which exists to be used is machinery that can be hijacked, and machinery that can be hijacked can sometimes be borrowed.** Hijacking is documented: RAG off-target activity drives oncogenic deletions in ETV6::RUNX1 ALL, and AID is genetically required for MYC-IgH translocation. Borrowing is what a fusion-junction-directed nuclease would be. And there is a third consequence that is purely defensive: because healthy cells make chimeric transcripts, a fusion transcript detected by RT-PCR is weaker evidence of a rearranged genome than the field usually treats it as.

Where KMT2A sits relative to all of this

KMT2A fusions are not made by any of the programmed DNA machineries. The junction architecture argues against recombinase involvement (mechanism-rag-off-target-recombination), and the initiating lesion is topoisomerase II cleavage, apoptotic nuclease activity or replication-stress-associated endonuclease G, resolved by error-prone end joining (mechanism-top2-cleavage-at-kmt2a, mechanism-apoptotic-nuclease-cleavage-kmt2a, mechanism-replication-stress-endog-cleavage-kmt2a, mechanism-nhej-vs-alt-ej-at-fusion-junctions). What KMT2A shares with the programmed systems is the joining chemistry (NHEJ and alt-EJ) and, with meiosis, the covalent intermediate chemistry. Those two shared layers are where a transferable intervention would have to act, and both are essential, which is the central difficulty.

Three conclusions the PI can use
  1. Prevention by suppressing breakage is not available. The break-making and break-joining enzymes are essential; suppressing them produces immunodeficiency, infertility or genome instability. There is no 'less breaking' drug.
  2. The separability result is the one to steal. AID-driven translocation requires AID and UNG but not H2AX, 53BP1 or Ku80, while normal switching does require them. If an analogous factor exists that is needed for KMT2A mis-joining but not for topoisomerase II function, it would be a genuine prevention target. Nobody has looked.
  3. Diagnostic humility. RNA-level fusion detection has a real, quantified false-positive class in paediatric ALL. Genomic confirmation should be required wherever a positive result would change management, and especially in any newborn-screening scheme (target-newborn-screening-fusion-transcripts).
How this could bite us back
  • The general lesson is unfavourable to prevention: every programmed-break pathway is essential, so nothing can be turned off without cost, and the KMT2A break does not even use those pathways.
  • The general lesson is favourable to caution about editing: vertebrate evolution needed a >1,000-fold suppression of transposition to make a programmable nuclease tolerable in lymphocytes [E1], and even then it drives recurrent oncogenic deletions [E5].
Still open
  1. Does a factor exist that is required for KMT2A mis-joining but dispensable for topoisomerase II's physiological function? This is the highest-value unexplored prevention question in the whole beat, and the AID/UNG-versus-H2AX result [E2] is the proof of concept that such factors can exist.
  2. Is there any physiological context in which a KMT2A chimeric transcript is produced deliberately? Nobody has looked in fetal haematopoietic tissue, where the leukaemia arises.
  3. Could the RAG machinery be deliberately redirected as a therapeutic, rather than merely feared as a hazard? It is the only programmable recombinase that is already native to the lymphoid lineage and therefore does not need to be delivered as a foreign protein.

The V(D)J recombinase is a domesticated transposase whose transposition activity vertebrates suppressed more than a thousand-fold, establishing programmed genome breakage as an engineered-risk system rather than a safe one.

    What this evidence cannot show

Comparative structural biology; the evolutionary account has a published alternative (PMID 34394111) and should not be presented as settled.

Evidence that argues against this (1)
    • The direct measurement has overtaken the analogy. This entry uses RAG domestication to argue that programmed genome breakage is an engineered-risk system and, by extension, that a fusion-junction-directed nuclease would be borrowing a dangerous tool. Leibowitz et al. now show the danger directly for the tool actually on offer: CRISPR-Cas9 editing generates micronuclei and chromosome bridges that initiate chromothripsis, demonstrated by single-cell whole-genome sequencing in model cells and by editing at a clinically relevant locus in clinically relevant cells. The 'borrowing' half of this entry's argument should be re-grounded on that result rather than on an evolutionary analogy, which makes the conclusion firmer and the reasoning shorter. A programmed-break pathway's oncogenic output can be genetically separated from its physiological output: AID-driven MYC-IgH translocation requires AID and UNG but not H2AX, 53BP1 or Ku80.

    What this evidence cannot show

Mouse genetics; the separability is partial and the 'not required' calls have limited power for partial effects.

Evidence that argues against this (1)
    • A second laboratory measured the same separability and reached the opposite conclusion about the AID half of it. Translocations between the Igh switch region and Myc were measured in AID-deficient mice, and AID made no measurable contribution to the generation of the INITIAL translocations - only to the outgrowth of translocation-positive cells. If AID is not required for the breakage-and-joining event, then 'requires AID and UNG but not H2AX, 53BP1 or Ku80' is not a clean genetic dissection of a programmed pathway's oncogenic output; it may be a dissection of what lets a translocated cell expand. Programmed meiotic double-strand breaks are made by a topoisomerase-like enzyme through a covalent protein-DNA intermediate, the same chemistry that initiates KMT2A breakage.

    What this evidence cannot show

Yeast; the chemical analogy to TOP2 is real but SPO11 and TOP2 differ in regulation, partners and adduct-resolution pathway.

Evidence that argues against this (1)
    • 'The same chemistry' holds at the level of the covalent intermediate and breaks down at the level of the enzyme. Meiotic break formation requires a TopoVIB-Like partner subunit, identified in mouse and plants as an essential conserved family: the meiotic machine is a two-subunit type IIB topoisomerase-like complex, whereas TOP2 is a homodimeric type IIA enzyme with no equivalent partner requirement. The 1997 assignment, made on sequence homology in yeast, could not see this. So the analogy that licenses reading meiotic break biology across to KMT2A cleavage is narrower than it looks - a shared 5'-phosphotyrosyl intermediate, not a shared enzyme or a shared regulation. In childhood B-precursor ALL RNA-seq, the large majority of candidate fusion calls are normal conjoined genes rather than rearrangements.

    What this evidence cannot show

    10 cases; caller- and reference-dependent.

Evidence that argues against this (1)
    • The defensive conclusion drawn here is right but its mechanism attribution is partly wrong. This entry says healthy cells make chimeric transcripts by trans-splicing and read-through, so an RT-PCR fusion signal is weak evidence of a rearranged genome. Yan et al. measured, using a cross-species RNA spike-in, that approximately 1% of RNA-seq reads are artifactually chimeric, produced by random template switching during reverse transcription rather than by any cellular process, and that real events are distinguished by reproducing the same chimeric fragment across independent biological isolates. A substantial share of the chimeric signal the field attributes to healthy-cell biology may be library chemistry. The practical instruction is unchanged and in fact simplified: confirm at the genome, and require cross-isolate reproducibility of any RNA-level chimera claim. Read: abstract only. Yan B et al. 2021

Off-target activity of a physiological recombinase is the predominant driver of oncogenic rearrangement in a common childhood leukaemia, demonstrating that programmed machinery does get hijacked.

RAG-mediated recombination is the predominant driver of oncogenic rearrangement in ETV6-RUNX1 acute lymphoblastic leukemia · Papaemmanuil E, Rapado I, Li Y, Greaves M, Campbell PJ, et al. · Nature Genetics 2014
    What this evidence cannot show

Applies to ETV6::RUNX1 and explicitly not to KMT2A.

Evidence that argues against this (1)
    • Searched and empty, recorded as a fact. The generalisation from E5 -- RAG off-target activity drives oncogenic rearrangement in ETV6::RUNX1 ALL, therefore programmed machinery gets hijacked -- is not established for KMT2A. This pass found no primary report that RAG or AID activity generates KMT2A breaks; the mechanism attributed to KMT2A breakage in this graph is topoisomerase-II-mediated cleavage, which is chemistry shared with SPO11 (E3) but is not a lymphoid programmed-recombination pathway. The entry's strategic claim therefore rests on an analogy across fusion families and should say so. No citation is attached because nothing was found to cite.
