Why this one rare leukaemia was thought to decide an argument about a drug used in a commoner one - and why it decides less than the title says (corrected 2026-09-21)

also called the CALM-AF10 natural experiment

To know whether a drug works because it blocks a specific handshake between two proteins, you want a leukaemia that does everything else the same way but has no handshake to block. CALM-AF10 looked like that leukaemia. But two other leukaemias with no handshake of their own turn out to respond to the drug anyway, so a negative result in CALM-AF10 would no longer prove what this entry originally said it would.

The same thing, for a scientist

A layman-first explainer for why a fusion found in a few percent of T-ALL patients matters to a hypothesis about infant B-ALL. Menin inhibitors are approved for KMT2A-rearranged leukaemia, and the accepted account is that they work by prising the fusion protein off menin. But almost everything else about a KMT2A-rearranged cell - the HOXA/MEIS1 growth programme, the DOT1L dependence, the blocked maturation - is shared with **PICALM::MLLT10**, which contains no KMT2A and therefore no handshake. **If CALM-AF10 cells ignore a menin inhibitor, the handshake account is confirmed and sharpened. If they die, the account is wrong and menin is doing something more general.** That is the shape of a control experiment, and nature has already set it up. **CORRECTION, 2026-09-21 (Q67).** The clean either/or in the sentence above does not hold, and the open question at the foot of this entry - is there a HOXA-high leukaemia that is menin-dependent WITHOUT a menin-binding motif - has an answer that was published before this entry was written. **NPM1-mutant AML carries no fusion at all and responds to revumenib**, and **NUP98-rearranged AML carries no KMT2A and is a menin-MLL1 dependency** with VTP50469 active at an IC50 the authors report as similar to KMT2A-rearranged and NPM1c cells, MLL1 and the NUP98 fusion evicted from chromatin together, and survival prolonged in NUP98-NSD1 and NUP98-JARID1A xenografts. So menin dependence does not track the presence of a menin-binding motif in the fusion, and 'contains no KMT2A' is not the independent variable this control needs it to be. **The experiment is still worth running and its interpretation has narrowed: a positive result would place CALM-AF10 alongside NPM1c and NUP98 rather than refuting the handshake account, and only a negative result is now strongly informative.**

Go deeper
The argument in five steps
  1. A KMT2A fusion is two things stuck together: an N-terminal piece of KMT2A that decides where on the genome to sit, and a partner that decides what to do when it gets there. The 'where' includes a grip on menin.
  2. Menin inhibitors break that grip. Revumenib is approved on exactly that rationale.
  3. CALM-AF10 has the 'what to do' and not the 'where'. It carries the same AF10 module, which grabs the same DOT1L enzyme through the same physical contact - but its other half is a clathrin adaptor with no grip on menin and no CpG-island targeting module.
  4. Yet it produces the same output: HOXA5/7/9/10 and MEIS1 switched on, maturation blocked, and an absolute requirement for DOT1L.
  5. So the question becomes sharp. Is menin needed because the HOXA programme needs it, or because a KMT2A fusion needs it? These have been treated as the same statement. They are not, and CALM-AF10 separates them.

Step 5 does not follow as cleanly as written, and this is the correction added 2026-09-21. The step assumes that menin dependence requires a menin-binding motif in the driving oncoprotein. Two counter-examples are published: NPM1-mutant AML has no fusion and responds to menin inhibition clinically, and NUP98-rearranged AML has no KMT2A and is menin-MLL1-dependent in vitro and in xenografts, through WILD-TYPE MLL1 rather than through a fusion-borne motif. Wild-type MLL1 is present in every cell. So the corrected form of step 5 is: is menin needed because a KMT2A FUSION needs it, or because the HOXA programme needs an MLL1 that menin holds on chromatin - whether that MLL1 is fused or not? CALM-AF10 still separates those two, but only in one direction, because a positive result is now the expected one.

What makes this more than a thought experiment

Three things. The cells exist - U-937 is a standard laboratory line carrying this fusion, sold by every cell bank. The drug exists and is approved. And nobody has done it: as of 2026-09-20, joining any CALM-AF10 name to any menin name returns zero published records, with the operand controls run to prove the search works. The experiment is two weeks and the answer changes what this project believes either way.

There is a second instance, which makes the test stronger rather than merely repeating it: SET::NUP214, another T-ALL fusion with no KMT2A that also turns on HOXA and also works with DOT1L and CRM1. Run in the same plate, two unrelated lesions would answer the same question twice.

And the second instance is weaker than it looked. SET::NUP214 interacts with wild-type MLL via the SET acidic region of the fusion, and the two cooperatively enhance the HOXA10 promoter - so that lesion reaches HOXA through the very protein menin binds. Two KMT2A-free lesions that both route through MLL1 are one argument, not two, and the argument points toward menin involvement rather than away from it. Running both in the same plate is still the right design; describing it as answering the same question twice with unrelated lesions is not.

The honest caveat a lay reader should carry away - now stronger than when this entry was written

Nobody has run this. Everything above is a prediction from three indirect lines - the fusion's architecture, one cell line in a public dependency screen, and a chromatin screen performed in mouse MLL-AF9 cells. The graph's position is probably menin-independent, untested, and cheap to settle - not menin-independent.

Updated 2026-09-21: 'probably' is no longer defensible. Two published counter-examples show a leukaemia can be menin-dependent with no menin-binding motif of its own - NPM1-mutant AML, treated clinically on that basis, and NUP98-rearranged AML, where the dependency runs through wild-type MLL1. Chromatin-bound CRM1 recruits SET-Nup214 AND NPM1c to the HOX clusters, so the CRM1 address this graph treats as the KMT2A-free route is shared with a lesion that IS menin-dependent. And the revumenib protocol's HOX/MEIS cohort, which this entry already flagged as the sponsor betting the other way, now looks less like a bet and more like the reading of the evidence. The honest position is: untested, cheap to settle, and the prior has moved toward menin dependence rather than away from it.

How this could bite us back
  • Explaining a hypothesis clearly makes it easier to believe. This one is a prediction with no direct evidence behind it, and the clarity of the argument is not evidence.
Still open
  1. Is CALM-AF10 menin-dependent? Still unrun, still two weeks, and now with a published prior pointing at yes rather than no.
  2. If CALM-AF10 turns out menin-INdependent, that would be the interesting result - it would make it the only HOXA-high leukaemia tested that is NOT menin-dependent, and would need an explanation that NPM1c and NUP98 fusions do not supply.
  3. How do NPM1c and NUP98 fusions reach menin without a menin-binding motif? Both appear to act through wild-type MLL1. Whether CALM-AF10 engages wild-type MLL1 at all has never been asked, and it is a single co-immunoprecipitation.
  4. Chromatin-bound CRM1 recruits both SET-Nup214 and NPM1c to HOX clusters. Does it recruit wild-type MLL1 too? If so, the CRM1 address and the menin interface are one assembly and the whole comparison arm collapses.

CALM-AF10 drives the HOXA/MEIS1 programme and is absolutely dependent on DOT1L, in mouse models, while containing no KMT2A sequence.

    What this evidence cannot show

Murine models. The human evidence for the HOXA programme is patient expression profiling rather than perturbation.

Evidence that argues against this (1)
    • PUBLISHED CONFLICTING RESULT, and it is the objection that undoes this entry's premise. 'Contains no KMT2A sequence' is the independent variable this concept rests on, and it does not isolate menin: NUP98 fusions contain no KMT2A and are a menin-MLL1 dependency - VTP50469 is active in NUP98-HOXA9 and NUP98-JARID1A lines at an IC50 the authors report as similar to KMT2A-rearranged and NPM1c cells, evicts MLL1 and the fusion from chromatin together, and prolongs survival in NUP98-NSD1 and NUP98-JARID1A patient-derived xenografts. NPM1-mutant AML carries no fusion at all and is menin-inhibitor responsive. So the experiment nature ran varies KMT2A presence, and menin dependence does not track KMT2A presence. The control is not the control this entry says it is. Heikamp EB et al. 2022

As of 2026-09-20, no published study joins CALM-AF10 to menin: PubMed returns 0 for every combination, against 81 for CALM-AF10 alone and 2,237 for menin alone, with two same-shape positive controls returning 6 and 17.

    What this evidence cannot show

Bounds the published literature only. Unpublished cell-panel screening almost certainly exists.

Evidence that argues against this (1)
    • AUGMENT-101 cohort 2D enrols on HOX/MEIS upregulation regardless of genotype, so the sponsor's working hypothesis is the opposite of this entry's, and any patient treated under it is an unreported instance of the experiment. ClinicalTrials.gov 2026

SET-NUP214, a second KMT2A-free T-ALL fusion, binds HOXA promoters where it interacts with CRM1 and DOT1L, and its knockdown abolishes HOXA expression and induces differentiation in LOUCY cells.

    What this evidence cannot show

Paediatric T-ALL plus one cell line. Included as the second instance, not as a substitute for the first.

Evidence that argues against this (1)
    • PUBLISHED CONFLICTING RESULT: the second instance is weaker than the first, not a confirmation. SET-NUP214 interacts with MLL via the SET acidic region of the fusion, and the two cooperatively enhance HOXA10 promoter activity, with neither the SET region nor the NUP214 region alone sufficient. Wild-type MLL1 carries the menin-binding motifs. So the second KMT2A-free lesion reaches HOXA through the very protein that menin binds, and citing it as a second independent instance of KMT2A-free HOXA activation double-counts an argument that in this case runs THROUGH the KMT2A gene product. Two instances that both route through MLL1 are one instance, and it points at menin rather than away from it. Cigdem S et al. 2021
